Requirements
  • Hold an active Community Health Worker certification in Texas.
  • Be bilingual in spoken and written English and Spanish.
  • Complete and maintain all CMS-mandated training and state-applicable requirements, including competencies in interpersonal building, service coordination, and professional ethical conduct.
  • Have at least 3 years of relevant community outreach, facilitation, or healthcare experience.
  • Have strong computer skills and the ability to navigate web-based and app-based systems.
  • Understand the socio-economic and public health challenges facing disenfranchised populations in Texas.
  • Have written and verbal communication skills and the ability to motivate and influence different types of people.
  • Have a high school diploma or GED; a college degree is preferred.
  • Have a valid driver’s license, reliable transportation, and a flexible schedule for occasional work outside regular business hours.
  • Commit to a drug-free workplace and complete pre-employment substance abuse testing and background checks.
  • Safely and consistently drive to public places and member homes within a 50-mile radius in the listed locations.
  • Work independently in a remote team environment with minimal oversight.
  • Frequently carry up to 30 pounds of supplies and stand or walk for the duration of home visits or member interactions.
  • Use a computer, tablet, and smartphone to update data accurately in multiple secure systems.
Responsibilities
  • Execute person-centered care plans for patients with serious, high-risk illnesses and assist in monitoring and revising disease-specific plans, especially when frequent medication or treatment-regimen adjustments are required.
  • Act as the link between the patient and billing practitioner by supporting care coordination, managing provider availability, rescheduling, and providing post-hospital discharge support.
  • Conduct person-centered assessments to understand patients’ social and cultural contexts, identify unmet social needs affecting management of serious conditions, and develop specific action plans to address those needs.
  • Help patients and caregivers navigate the Texas healthcare landscape; teach self-advocacy skills for communicating with specialists and coordinate transportation or access to telehealth.
  • Support patient communication templates and automation; coordinate with the tech team to set up workflows, troubleshoot technical issues, and update patient charts in the Case Management System accurately.
  • Refer patients to supportive services and community-based resources, and establish and maintain partnerships with local Texas organizations to connect members with targeted resources such as legal aid and food assistance.
  • Use motivational interviewing and capacity-building to help patients manage chronic-illness stress, provide tailored education to improve treatment adherence, and offer emotional support to families and caregivers.
  • Maintain detailed logs of interactions, including time spent and activities performed, and ensure documentation meets Medicare clinical and social integration service standards.

  • Experience navigating high-risk chronic conditions or complex care management is a major plus.
  • A college degree is preferred.

Wider Circle joins with Highmark Wholecare to launch peer-led community health program. - Neighbor-to-Neighbor Model Aims to Improve Health Outcomes for Medicaid Members PITTSBURG (August 31, 2026) - Wider Circle, a company focused on building communities to improve health and quality of life through trusted connections, recently joined with Highmark Wholecare to launch a proven peer-led, community-based health program for its Medicaid members. The initiative launched Aug. 3 in six counties in the Lehigh-Capital and Southwestern regions of the Commonwealth - Beaver, Lancaster, Lehigh, Washington, Westmoreland and York. Approximately 5,000 Highmark Wholecare members are expected to be supported by the program. Wider Circle acquires Menda Health to expand group-based care for seniors with chronic pain

Wider Circle, a company focused on building communities to improve senior health, has acquired Menda Health, a virtual care provider for adults with chronic pain. The acquisition unites two organisations centred on group-based care models for seniors. Menda Health developed a group-based behavioural care model combining structured sessions with evidence-based approaches to help seniors manage chronic pain. The company was founded by Dr Les Aria, Arun Nijhawan and Kartik Shastri. Wider Circle's peer-driven model recruits community members to lead health engagement programmes for Medicare Advantage, Medicaid and dual-eligible members. The acquisition will enable Wider Circle to expand its longitudinal care capabilities. Financial terms of the deal were not disclosed.
