Requirements
  • Provide direct nursing care appropriate to residents' age-related needs.
  • Possess, at minimum, a Nursing Degree from an accredited college or university, or graduate from an approved LPN/LVN program.
  • Possess a current, unencumbered, active license to practice as an LPN/LVN in the state.

Responsibilities
  • Provide direct nursing care to residents and supervise the day-to-day nursing activities performed by nursing assistants.
  • Direct the day-to-day functions of nursing assistants and ensure nursing personnel comply with facility policies, procedures, job descriptions, and applicable regulations.
  • Review department policies, procedures, manuals, and job descriptions and recommend revisions.
  • Plan shift services, programs, and activities with assigned nursing staff and support personnel.
  • Maintain the Nursing Service Procedures Manual and ensure nursing service personnel follow it.
  • Prepare written and oral reports and recommendations concerning shift activities.
  • Coordinate nursing services with other resident services to maintain each resident's total regimen of care.
  • Participate in the nursing service quality assurance program and facility surveys.
  • Review, update, and assist with planning residents' discharge plans.
  • Interpret department policies and procedures for personnel, residents, visitors, and government agencies.
  • Admit, transfer, and discharge residents; complete related records, forms, reports, and documentation.
  • Protect resident and facility information by maintaining password confidentiality, safeguarding protected health information, reporting unauthorized access, and logging off workstations.
  • Complete admission, transfer, discharge, charting, accident, incident, and medical-record documentation.
  • Receive telephone orders from physicians and transcribe orders to resident charts, medication cards, and treatment and care plans.
  • Chart nurses' notes and accident or incident reports and report discrepancies to the Nurse Supervisor.
  • Record diet-order changes and forward them to Food Services.
  • Prepare and administer physician-ordered medications and treatments while verifying resident identity and medication accuracy.
  • Maintain medication, narcotic, supply, and equipment inventories and records and report discrepancies or needs.
  • Review medication cards for completeness, accurate transcription, and compliance with stop-order policies.
  • Notify physicians of automatic stop orders and dispose of drugs and narcotics according to procedure.
  • Participate in employee performance evaluations, determine shift staffing requirements, and make staffing and personnel recommendations.
  • Report staffing shortages and absentee call-ins to the Nurse Supervisor.
  • Develop work assignments, assist with assignments, provide leadership to assigned nursing personnel, and conduct daily rounds.
  • Ensure nursing assistants are enrolled in or have graduated from approved training and that nurse aide trainees work under direct licensed-nurse supervision.
  • Meet regularly with nursing personnel to identify and correct problem areas and improve services.
  • Receive and give nursing reports at the beginning and end of shift duty.
  • Report occupational exposures, suspected fraud, and safety or security incidents according to facility procedures.
  • Inform personnel of new admissions, prepare rooms, greet and orient residents and families, and escort residents as needed.
  • Make physician rounds, arrange diagnostic and therapeutic services, consult physicians, and review charts for treatments, medications, and diets.
  • Implement nursing objectives and standards and monitor prescribed treatments and residents' physical and emotional status.
  • Coordinate social and activity programs with nursing schedules and notify physicians and next of kin of accidents or changes in condition.
  • Carry out restorative and rehabilitative programs, including self-help and care.
  • Provide catheterization, tube feedings, suction, dressing and bandage changes, colostomy and drainage-bag care, specimen collection, vital-sign measurement, range-of-motion exercises, and other professional nursing services as required.
  • Use restraints according to policy, monitor seriously ill residents, and check residents who cannot call for help.
  • Inspect treatment areas, resident rooms, and nursing work areas for cleanliness and safety.
  • Check food brought by residents' families or visitors against dietary allowances.
  • Ensure direct care follows residents' care plans and wishes and review care plans for appropriate goals, problems, approaches, and revisions.
  • Meet with residents and family members and report problem areas to the Nurse Supervisor.
  • Arrange transportation for discharged residents, escort residents to pickup areas, notify families of deaths, contact funeral homes when requested, and follow post-mortem procedures.
  • Develop, plan, conduct, and schedule in-service training; orient new employees; standardize work methods; and train personnel on blood and body-fluid exposure tasks.
  • Assist with clinical supervision for nurse aide trainees and participate in outside, annual facility, advance-directive, and continuing-education programs.
  • Monitor personnel compliance with safety regulations, dress codes, infection-control procedures, hand hygiene, fire and disaster drills, and use of protective equipment.
  • Assist with identifying and recording tasks involving blood or body-fluid exposure and ensure required training and personal protective equipment are available.
  • Participate in developing and maintaining infection-control and hazardous-condition reporting procedures.
  • Maintain clean and sanitary nursing areas and ensure safe operation and efficient use of nursing equipment and supplies.
  • Recommend unit equipment and supply needs, maintain adequate stock, restrict equipment operation to trained and authorized personnel, and maintain required material safety data sheets.
  • Provide data to the Quality Assurance and Assessment Committee and assist with the Minimum Data Set and Resident Assessment Protocol care-plan process.

PACS Group has announced that co-founder and chief financial officer Mark Hancock will retire on 30 June 2026. He will continue serving on the company's board of directors as vice chairman. Dr Conway currently serves as CEO of Optum, UnitedHealth Group's health services division with revenues exceeding $200 billion.
