View

Hopscotch Health delivers value-based primary care to underserved rural communities in America. It partners with healthcare providers and payers to offer preventive care and chronic disease management aimed at improving patient outcomes and satisfaction while reducing overall costs. The service works through a value-based care model that emphasizes care coordination and prevention, collaborating with rural clinics to deliver coordinated primary care rather than maximizing visit volume. Its differentiator is a focused commitment to rural health disparities, bringing a proven urban-value-based approach to areas with limited healthcare infrastructure, supported by provider-payer partnerships. The company’s goal is to close rural healthcare gaps by delivering accessible, high-quality, cost-effective primary care that improves health outcomes for patients at risk of heart disease, cancer, stroke, respiratory issues, and injuries.
