Previous MDS Coordinator experience in a skilled nursing facility.
Strong knowledge of MDS 3.0 and Resident Assessment Instrument processes.
Understanding of Medicare, Medicaid, and skilled nursing facility reimbursement requirements.
Strong organizational skills and attention to detail.

Responsibilities
Coordinate and oversee the MDS assessment process.
Ensure assessments are accurate, timely, and compliant with current Centers for Medicare & Medicaid Services requirements.
Collaborate with Nursing, Therapy, Social Services, Dietary, and other interdisciplinary team members.
Lead and facilitate care-plan development and coordination.
Monitor documentation and identify opportunities to improve clinical outcomes.
Support regulatory compliance and survey readiness.
Help ensure the interdisciplinary team works together toward the best outcomes for residents.
Maintain certifications and recertifications.
Initiate death certificates.

WHAT YOU’LL DO
