- An Associate’s degree in Nursing with relevant experience is accepted.
- Five years of experience as a Registered Nurse or Registered Nurse Case Manager providing complex care management is required.
- A minimum of three years of experience in medical-surgical nursing or home health is required.
- Experience providing transitions of care is required.
- An unencumbered Registered Nurse license or compact nursing license is required, or a compact nursing license must be obtained within six months of hire.

- The candidate must have verbal and written communication skills, organizational skills, and attention to detail.

- The candidate must be able to use documentation tools and technological platforms, including electronic medical records, and be comfortable with digital technologies.
- The candidate must provide respectful, professional customer service and be able to make solution-oriented decisions.
- Work with the clinical department and act as a liaison with physician practices.
- Advocate for personalized treatment options that address each patient’s care needs.
- Create and manage plans of care for patients with chronic or serious conditions.
- Use assessment and communication skills to engage patients needing clinical support, determine and prioritize their needs, and provide patient-centered care within the scope of practice.
- Provide evidence-based education, assessment, and care navigation.
- Identify patient and caregiver education needs through telephonic assessment and engagement.
- Ensure patients and caregivers have adequate information to support successful transitions home from inpatient or post-acute facility stays.
- Conduct timely telephonic clinical outreach to identified patients.
- Collaborate with primary care physicians, nurse practitioners, and other healthcare team members to coordinate care and help keep patients stable at home.
- Serve as the point of contact and informational resource for patients, care teams, families and caregivers, payers, and community resources.
- Implement interventions that improve health outcomes, lower costs, and enhance the patient experience.
- Work with provider offices, skilled nursing facilities, hospitals, and other Clinical Services teams to support patient needs.
- Coordinate care across the continuum while maintaining confidentiality.
- Guide patients through the healthcare system and help them overcome barriers.
- Coordinate treatment and services for patients.
- Schedule medical appointments as needed.
- Communicate with patients and their families about patients’ health conditions.
- Provide community resources and support resolution of social determinants of health.
- Maintain comprehensive working knowledge of community resources.
- Assume accountability for the quality of care.
- Pursue new knowledge and learning that supports clinical care coordination.
- Travel minimally, when required by the market location, to visit provider offices and enhance provider-office engagement.
- A Bachelor’s degree in Nursing is preferred.
- Startup experience is preferred.
- An Accredited Case Manager certification is preferred.

The Shaker Schools Foundation recently announced its Alumni Hall of Fame Class of 2026, which will be celebrated with a Hall of Fame dinner and induction ceremony 6 p.m. Oct. 10 at Shaker Heights Country Club at 3300 Courtland Blvd. Colson Baker, better known by his stage name mgk, Matt Buder Shapiro, Richard Lash, Thomas Miller, Tiffany Mizen, Sanford Watson and Lorna Wisham are the seven inductees set to join the over 345 graduates who have been placed in the hall since it was established in 1986. Baker, class of 2008, is a Grammy-nominated multi-platinum recording artist who has had two albums hit No. 1 on the Billboard 200. His latest studio album, "Lost Americana," was released in 2025 and marked his seventh overall top 10 and third-consecutive No. 1 rock and alternative album. Baker also founded The 27 Club coffee shop in the Flats and hosts "mgk day weekend" every August. Buder Shapiro, class of 2008, is the CMO and a partner at Denver-based Ensemble Performing Arts, a community of more than 130 music and dance studios and 2,500 teachers across the United States. He was previously the CMO and head of growth at Hoboken, N.J.-based Vytalize Health, where he helped scale the company to more than $3 billion in revenue. The New Jersey-based company aids physician practices and health centers in transitioning to value-based care through its ACO model.
