Requirements
- An advanced degree in Nursing (ARNP or similar) is required.
- Current Nurse Practitioner certification in the state of practice is required.
- A minimum of 3–5 years of primary care patient panel ownership experience is required.
- A full practice authority Illinois license is required.
- A current DEA number for Schedule II–V controlled substances may be required based on the state of practice.
- Basic Life Support certification from the American Heart Association or American Red Cross is required within the first 90 days of employment.
- Demonstrated record of consistently achieving clinical performance metrics in the current role.
- Basic computer skills and some knowledge of Microsoft Office Suite.
- The position may require 50–75% local travel.
Responsibilities
- Provide direct patient care in medical centers, nursing homes, skilled nursing facilities, and home settings, or conduct assessments in skilled nursing facility and home settings, depending on the assignment.
- Independently assess acute and non-acute clinical problems; perform and document physical assessments and patient histories; analyze trends in patient conditions; and develop, document, and implement patient management plans based on the information obtained.
- Assist in developing nursing plans of care and provide appropriate counseling and education to patients, families, and significant others.
- Plan patient care based on knowledge of the patient population or protocol, anticipate and identify commonly encountered physiological and psychological problems, consider patients’ cultural backgrounds, understanding, personalities, and support systems, and serve as a patient advocate.
- Write admission, transfer, and discharge orders.
- Order and interpret appropriate laboratory and diagnostic studies, and order appropriate medications and treatments.
- Refer patients for indicated consultations, including dermatology, neurology, ophthalmology, endocrinology, surgery, intensive care, infectious disease, hematology, psychiatry, social services, and dietary services; document care in detailed progress notes and summaries.
- Perform invasive procedures independently after documented competency is completed.
- Participate in patient care rounds and conferences, communicate patient management strategies to the care team, and collaborate with multidisciplinary team members to meet patient care needs.
- Recognize situations requiring immediate physician attention and initiate life-saving procedures when necessary.
- Use advanced communication skills to solve complex situations and improve processes and patient services.
- Collaborate with multidisciplinary team members to evaluate healthcare delivery systems and identify and implement new practice patterns as appropriate.
- Initiate arrangements and write orders for skilled nursing facility discharges, and complete the appropriate paperwork.
- Work with physicians, nurses, physical therapists, social workers, families, and key caregivers to transition patients to a lower level of care when medically appropriate.
- Introduce the nurse practitioner role to patients and families.
- Facilitate patient and family conferences to review treatment goals, optimize resource use, provide family education, and identify post-skilled-nursing-facility needs.
- Support informed patient and family decisions about goals of care, palliative care, and hospice.
- Facilitate discharge to the appropriate level of care and use preferred providers when additional services are required.
- Prescribe medication according to the state of practice.
- Adhere to departmental objectives, performance standards, regulatory and quality requirements, and applicable policies and procedures.
Desired Qualifications
- Spanish language skills are highly preferred.
- Board certification by AANP or ANCC is preferred, though it may be required in certain states.
