Requirements
  • A minimum of two years of experience in a Residential Counselor role.
  • A demonstrated track record as an exceptional performer and team player in a Residential Counselor role.
  • A high school diploma or equivalent is required; in some cases, experience may be substituted for academic training.
  • A valid driver's license, adequate insurance when using a personal vehicle, and a safe driving record are required.
  • Must be at least 21 years of age and have maintained a valid United States driver's license for at least six months.
  • Must pass a driver's screening background check.
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, climb stairs, sit, reach, and maneuver through narrow spaces or hallways.
  • Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift.
  • Ability to operate a computer and office equipment such as a calculator, copier, and printer.
  • Ability to assist clients with activities of daily living.
  • Ability to speak, hear, and communicate with clients, staff, and external representatives.
  • Required vision abilities include close, distance, color, peripheral, and depth perception, as well as the ability to adjust focus.
  • Ability to maintain confidentiality and work independently and as part of an interdisciplinary team.
  • Ability to use advocacy, empowerment, self-advocacy, participatory planning, crisis intervention, household management, and physical personal management techniques.
  • Ability to learn and use online tools such as Microsoft Outlook and an electronic health record.
  • CPR certification is required within two weeks of hire.
  • First Aid certification is required within two weeks of hire.
  • Medication Administration Certification follows Connecticut guidelines.
  • Safety Care certification is required within two weeks of hire.
Responsibilities
  • Assist in developing, implementing, and documenting treatment, including Individual Service Plans, Individual Plans, and clinical programs.
  • Provide training and support to persons served, including rehabilitative and behavioral interventions, monitoring, fire and safety regulations, appointment scheduling, money management, activities of daily living, grocery shopping, meal planning and cooking, household chores, and related activities.
  • Perform hands-on tasks in the program as required to implement support plans.
  • Assist individuals with community-based support and resource services and help them become integrated into their communities.
  • Plan recreational and social activities, outings, community travel, church activities, shopping, celebrations, and other activities that promote emotional well-being and quality of life.
  • Provide transportation as necessary.
  • Assess residents' physical and emotional status and report changes to a supervisor.
  • Monitor residents' funds as needed.
  • Maintain records for persons served, collect behavioral and academic data, maintain progress notes, enter information into the electronic health record, and document incidents, programs, and routine activities in communication logs.
  • Store, administer, document, and dispose of medication consistent with Medication Administration Certification requirements, funding-source regulations, and company policy.
  • Communicate with and provide information to program nurses, funding agencies, case managers, other service providers, Visiting Nurses Association nurses, and physicians.
  • Participate in mandated training and staff meetings as directed by the manager.
  • Assist with the orientation of new staff and ensure required staff-to-consumer ratios are maintained, including providing last-minute coverage when required.
  • Ensure compliance with internal and external regulatory bodies and corporate and divisional policies and procedures.
  • Ensure program site safety and maintenance, including housekeeping, lawn mowing, snow shoveling, and garbage removal.
  • Complete paperwork and online records, including communication logs, community membership logs, chore schedules, timesheets, and application forms.
  • Serve as a human rights advocate and provide physical intervention or first aid during behavioral or medical emergencies.
  • Report to the Disabled Persons Protections Commission when required.
  • Maintain required certifications, review and respond to work emails, and follow administrative policies.
