Requirements
  • Must hold a current Licensed Practical Nurse or Registered Nurse license in the state of practice.
  • Must have one year of acute care hospital experience.
  • Must have independent means of transportation, a valid driver's license, and the ability to drive.
  • Must maintain current automobile liability insurance coverage.
  • Must have computer skills, including knowledge of spreadsheet and word-processing software.
  • Must have clerical and business-machine skills and knowledge of business accounting.
  • Must have good organizational and personal communication skills.
Responsibilities
  • Respond to patient referrals within one hour of notification.
  • Visit patients to explain physician orders, services, and LECH procedures, and obtain information needed to facilitate transfer.
  • Complete accurate and comprehensive head-to-toe functional assessments of referred patients.
  • Verify address, phone number, and payer information; confirm eligibility criteria; and coordinate the organization and transfer of orders and referral information to the applicable agency or facility staff.
  • Review patient assessments with the Medical Director within one to two hours.
  • Notify the referral source of patient acceptance or denial within two hours of receiving the referral.
  • Obtain final Administrator and Director of Nursing approval for all recommended admissions.
  • Involve family members and caregivers in the education process and assess post-discharge education and coaching needs.
  • Coordinate post-facility discharge follow-up for patients with an anticipated shorter length of stay and assist LECH in preparing to accept the patient.
  • Participate in daily facility staff meetings to support patient satisfaction and continuity of care.
  • Serve on facility committees when requested and work with hospital focus groups on systems integration and process improvements that improve patient outcomes and transitions of care.
  • Participate in scheduled meetings with staff, the Director of Nursing, the Medical Director, and the Medical Advisory Board.
  • Attend monthly calls and company-provided in-service sessions when required.
  • Provide educational in-services about LECH features, benefits, and specialty programs, and educate staff about available services.
  • Develop and improve relationships with referring physicians and other entities in primary target markets.
  • Identify and educate new referral sources about the value of LECH services and programs.
  • Communicate with the growth team and analyze best practices and opportunities to reach underserved populations within service areas.
  • Develop and implement a census-growth plan to achieve the budgeted admission goal.
  • Maintain appropriate statistics and generate reports as required by facility policy and procedure.
  • Follow up on internal and external barriers related to the referral and admission process to ensure rapid resolution.
  • Review and provide written monthly analysis of denial tracking to the facility Administrator and recommend an action plan.
  • Promote LECH ethically and professionally at conventions, seminars, and professional meetings.
  • Implement new ideas to promote the facility and its services.
  • Maintain and promote community public relations through participation in promotional and marketing opportunities.
  • Practice performance management with the goal of increasing admissions.
  • Model and foster open communication among departments, teams, and individuals.
  • Allocate resources according to priorities.
  • Manage time responsibly.
  • Attend Referral Development meetings as directed by the Administrator.
  • Complete required expense paperwork and scheduled events in the Playmaker CRM database according to company policies.
  • Maintain confidentiality and ethical conduct when dealing with physicians, staff, patients, and families.
