Requirements
  • A Registered Nurse licensed to practice in New York State is required.
  • Specialized training in intellectual and developmental disabilities and/or one year of experience in community health care services is required.
  • The candidate must be an AMAP trainer.
  • The candidate must be trained in cardiopulmonary resuscitation and first aid.
Responsibilities
  • Assess individuals’ health conditions and determine the need for referral to a physician or emergency room.
  • Participate in pre-admission screening and evaluation of individuals referred to ICF or IRA programs.
  • Develop an initial nursing assessment within 24 hours of placement at a residence.
  • Develop a Plan of Nursing Services based on the nursing assessment, physical examination report, and evaluation of the individual’s ability to self-medicate within 30 days of placement and annually thereafter.
  • Ensure control of communicable diseases and infections through identification, assessment, reporting to medical authorities, and implementation of protective and preventive measures.
  • Periodically observe medication administration to ensure adherence to OPWDD regulations and agency policy.
  • Report all injuries to management immediately so that incident-reporting procedures can be followed.
  • Complete semi-annual and annual nursing assessment reports.
  • Assess mealtime needs for choking prevention, add plan changes to Plans of Nursing Services, and train program specialists and program or assistant directors.
  • Maintain a weekly schedule and nursing assignments.
  • Participate in the development, review, and update of Individual Program Plans as part of the interdisciplinary team process.
  • Review and update Nursing Service Plans semi-annually and as needed based on nursing evaluations and quarterly nursing physical assessments.
  • Ensure that nursing care prescribed by the physician or identified by individual needs is carried out.
  • Participate in the development of discharge plans.
  • Follow up on and process individuals’ hospital admissions and discharges from hospitals or rehabilitation facilities.
  • Make referrals to appropriate community resources based on the completed medical portion of the individual’s Community Service Plan.
  • Update medical diagnoses and changes in plans of care in the Precision Care electronic health record.
  • Complete quarterly Quality Improvement Reports and ensure resolution of outstanding issues.
  • Review and follow up on Quality Improvement Reports completed by management.
  • Participate in annual and semi-annual interdisciplinary team meetings and special meetings within or outside the residence.
  • Participate in the monthly review and reordering of individuals’ medications with the primary physician, psychiatrist, and residence Behavior Intervention Specialist.
  • Ensure proper storage and disposal of medications and medical supplies in accordance with OPWDD regulations and agency policy.
  • Review and monitor bowel movement records, vital signs, monthly weights, adaptive equipment, seizure activity and records, head injury questionnaires, menstrual records, and applicable flowsheets for health maintenance.
  • Report irregularities or abnormalities to the individual’s primary physician as needed.
  • Ensure ongoing health care services are provided as needed and medical appointments are scheduled in a timely manner.
  • Monitor nutritional services and ensure annual diet and consistency orders are signed by a health care practitioner.
  • Remain available 24 hours per day by cell phone according to medical emergency procedures, telephone triage protocols, and consultation needs.
  • Carry out agency policies issued by management in accordance with OPWDD and other funding-agency guidelines.
  • Attend state survey inspections.
