Requirements

  • CPR, First Aid, Medication Administration Plan, Safety Care, Positive Behavioral Supports Tier II, and NET certifications are required within the stated pre-hire or post-hire timeframes.
Responsibilities
  • Assist individuals in becoming integrated into their communities and developing and maintaining friendships.
  • Assist individuals in accessing community-based support and resources.
  • Plan recreational and social activities that promote social inclusion and emotional well-being.
  • Empower individuals to make choices, self-advocate, and participate in decisions about their supports and lives.
  • Support access to assistive technology and adaptive equipment.
  • Support individuals in securing, maintaining, and growing work, volunteer, and community-engagement opportunities.
  • Encourage individuals and their families to maintain strong partnerships with staff and other interested parties.
  • Provide administrative support to the Program Director, including scheduling staff assignments, training new staff, and assisting with clinical paperwork.
  • Function as designated lead staff for shift management and clinical programming.
  • Assist in developing, implementing, and documenting treatment plans, including ISP/PSTPs and clinical programs.
  • Provide training and support in rehabilitative and behavioral interventions, fire and safety regulations, appointment scheduling, money management, activities of daily living, grocery shopping, meal planning, cooking, household chores, and use of generic resources.
  • Perform hands-on tasks required to implement support plans.
  • Provide transportation as necessary.
  • Assess individuals' physical and emotional status and report changes to the supervisor.
  • Monitor individual funds as needed.
  • Maintain individual records, collect behavioral and academic data, maintain progress notes, and document incidents, programs, and routine activities in communication logs.
  • Procure, store, administer, document, and dispose of medication in accordance with MAP requirements, funding-source regulations, and company policy.
  • Communicate with program nurses, case managers, Visiting Nurses Association staff, and other interested parties.
  • Participate in mandated training and staff meetings.
  • Assist with orienting new staff.
  • Ensure compliance with internal and external regulatory policies, procedures, and standards.
  • Maintain home safety and upkeep, including housekeeping, snow shoveling, and trash removal.
  • Complete communication logs, community membership logs, activity schedules, and other required paperwork.
  • Provide shift coverage in other homes or programs as assigned.
  • Ensure required staff-to-individual ratios are maintained and assist the Program Director in securing coverage.
  • Function as a human rights advocate, provide physical intervention or First Aid during behavioral or medical emergencies, and report to the Disabled Persons Protection Commission.
  • Perform other related duties as required.
