Requirements
  • A Bachelor’s Degree in Social Work or a related field, or at least two years of college with demonstrated experience working with aging populations.
  • Knowledge of public benefits and entitlements for low-income seniors.
  • Proficiency in Microsoft Office.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Ability to work independently and collaboratively.
  • Bilingual ability in Spanish, Chinese, or Korean is preferred.
Responsibilities
  • Assess tenants and develop individual care plans.
  • Coordinate case management needs, collateral providers, natural supports, and care plan meetings.
  • Maintain regular contact with clients and coordinate with referred service providers.
  • Develop and facilitate counseling sessions as needed.
  • Provide advocacy and resource linkages.
  • Create curriculum and training to educate building staff on the unique needs of the target population.
  • Promote community and culture-building activities, including events, celebrations, presentations, training exercises, shopping trips, arts and crafts, tenant appreciation activities, holiday parties or lunches, and applicable fire drills.
  • Prepare written reports and maintain timely, accurate records and data collection.
  • Provide tenant customer service, including applying for benefits and entitlements, coordinating healthcare, assisting with rental money orders or checks, and ensuring tenant concerns are addressed promptly.
  • Complete initial intake, assessments, and file consent forms.
  • Complete updated assessments when medical or social changes occur.
  • Conduct periodic home visits and wellness checks for tenants with special needs or circumstances.
  • Maintain a daily case assistance log.
  • Complete detailed progress notes.
  • Complete detailed monthly reports for the New York City Department of Housing Preservation and Development.
  • Assist the Supervisor and/or Program Director as needed.
