Requirements
  • A minimum Associate's Degree is required.
  • Experience working with people living with AIDS, homeless people, or substance users is encouraged.
  • Effective written and oral communication skills are required.
  • Basic computer skills, including Microsoft Office suite and Windows, are required.
Responsibilities
  • Provide direct, field-based services to enrolled consumers.
  • Support the implementation of a comprehensive Service Plan for each enrolled consumer.
  • Assist consumers with securing housing, medical care, nutrition, and transportation services.
  • Assist consumers with applications and paperwork for benefits and support services.
  • Conduct regular social services and benefits reassessments and follow-ups.
  • Visit enrolled consumers between regular clinic visits to reinforce treatment plans, provide appointment reminders and individual health education, coordinate transportation and health care needs, and conduct other follow-up activities as warranted.
  • Accompany consumers to primary care, specialty care, and other health care appointments, as well as other social service appointments when warranted.
  • Provide field-based Directly Observed Therapy services, including observing and recording medication intake, monitoring compliance levels, and assessing and reporting side effects.
  • Provide critical feedback to other members of the health care team based on field observations.
  • Record all service provision for daily submission.
  • Facilitate groups for Care Coordination Redesign consumers.
  • Document all services in AWARDS Case Management Software.
  • Collect and record Primary Care Status Measures data as assigned.
  • Participate in daily planning meetings with the SRO Intervention Project Team and periodically participate in team meetings, case conferences, division meetings, and required trainings.
  • Participate in Services for the Underserved Quality Assurance and Quality Improvement activities.
  • Report to the Program Director in the absence of the Care Coordination Redesign Supervisor.
Desired Qualifications
  • Bilingual Spanish language ability is encouraged.
  • Experience working within an outreach model or in case management is preferred.

It coordinates services such as case management, housing and stabilization, behavioral health treatment, recovery supports, rehabilitation, advocacy, and community-based programs tailored to each person’s needs. By linking housing, health, justice, and community resources, S:US delivers integrated, place-based services that address multiple barriers at once rather than focusing on a single issue. The goal is to help people lead purposeful lives, improve neighborhoods, and break the cycle of poverty for current and future generations of New Yorkers.
