Requirements
  • Graduate of an accredited school of nursing.
  • One to two years of recent acute care experience in an institutional setting and one to two years of recent home care experience.
  • Current nursing licensure in the state.
  • A valid driver's license and an insured, properly maintained automobile meeting state and organizational requirements.
  • A bachelor's degree, with one year of hospice care experience preferred.
  • Excellent observation, verbal and written communication, problem-solving, and basic math skills, along with nursing skills demonstrated through the competency checklist.
  • Ability to walk or stand for prolonged or considerable periods; lift, position, and transfer patients; lift supplies and equipment; and perform reaching, stooping, bending, kneeling, and crouching as required.
  • Visual acuity and hearing sufficient to perform required nursing skills.
Responsibilities
  • Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs, including physical assessments and histories of current and previous illnesses.
  • Provide professional nursing care using the nursing process and according to the state Nurse Practice Act.
  • Write and initiate plans of care, regularly reevaluate patient and family or caregiver needs, and revise plans of care as necessary.
  • Use health assessment data to determine nursing diagnoses.
  • Develop care plans with goals based on nursing diagnoses and palliative nursing actions, involving patients and families in the planning process.
  • Initiate preventive and rehabilitative nursing procedures and administer medications and treatments prescribed in the physician's plan of care.
  • Counsel patients and families regarding nursing and related needs.
  • Provide health care instructions according to assessments and plans of care.
  • Assist patients with activities of daily living and support their efforts toward self-sufficiency and comfort care.
  • Act as case manager when assigned and coordinate care for an assigned caseload.
  • Complete, maintain, and submit accurate clinical notes regarding patient conditions and care provided, including pain and symptom management changes and outcomes.
  • Communicate with physicians about patient needs and condition changes and obtain or receive physician orders as required.
  • Communicate with community health-related personnel to coordinate care plans.
  • Teach patients and families or caregivers self-care techniques and provide medication, diet, and other instructions ordered by physicians.
  • Work with the interdisciplinary group to meet patients' and families' emotional needs and maintain continuity of care.
  • Provide and maintain a safe environment for patients.
  • Attend interdisciplinary group meetings.
  • Participate in on-call duties according to the on-call policy.
  • Ensure equipment and other necessary items and services are arranged and available.
  • Supervise ancillary personnel and delegate responsibilities when required.
  • Maintain and upgrade professional knowledge and practice skills through continuing education and in-service classes.
  • Fulfill requested or accepted case assignments.
  • Participate in quality assessment and performance improvement teams and activities.
