Requirements
  • A Bachelor's Degree in Pharmacy or a PharmD degree is required.
  • At least 1 year of clinical patient care and orders management experience is required.
  • A license to practice pharmacy in the State of Wisconsin must be obtained within 90 days.
  • Basic Life Support certification must be obtained within 90 days.
  • Advanced Cardio Life Support certification must be obtained within 180 days.
  • Pediatric Advanced Life Support certification must be obtained within 180 days.
Responsibilities
  • Provide medication therapy management services as part of the interdisciplinary team for patients on inpatient services to ensure drug therapy is safe, effective, efficient, and cost-effective.
  • Actively participate in bedside medical emergencies by providing expertise in medication selection, preparation, procurement, documentation, and medication history review.
  • Engage in interdisciplinary patient care rounds, including medication reconciliation, therapy monitoring, order verification, patient education, drug information support, and individualized drug dosing.
  • Provide patient care to critical care, medical, and surgical patients.
  • Teach and precept Pharmacy Residents and PharmD students through structured practical experiences and formal didactic education.
Desired Qualifications
  • Completion of an ASHP-accredited residency training program.
  • At least 3 years of institutional experience in a hospital or integrated health system, or completion of an ASHP-accredited residency training program.
  • Certification through an appropriate certifying program, such as the Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation Providers, or Specialty Pharmacy Certification Board.
