Requirements
  • Experience working with people with psychiatric disabilities, developmental disabilities, medical illnesses, and other co-occurring disorders is preferred.
  • A high school diploma or equivalent is required.
  • An Associate’s degree in Nursing is required.
  • A valid United States driver’s license is required if driving for Vinfen; drivers must be at least 21 years old, have held the license for at least one year, pass an annual driver-screening background check, and maintain a safe driving record.
  • Drivers using a personal vehicle must maintain adequate insurance.
  • The ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds using proper lifting techniques or a two-person lift, operate a computer and office equipment, assist clients with daily living activities, communicate with clients and representatives, and perform the specified visual and physical tasks is required.
  • For day program or group residence positions, the ability to assist with routine living activities, including cleaning, meal preparation, vacuuming, shoveling, and grocery shopping, is required.
  • CPR certification is required within two weeks of hire.
  • An LPN license in the state of service is required.
Responsibilities
  • Participate in comprehensive physical-health assessments upon admission and when a client’s medical status changes.
  • Provide nursing care, teaching, and assistance with daily living activities according to each client’s care plan, including wound care, diabetes care, mouth care, foot care, GT/JT care, nutritional management, and catheter care.
  • Monitor clients for changes in status and communicate those changes to the Program Manager, primary care provider, family or guardian, and other appropriate agencies.
  • Perform individualized assessments and provide skilled nursing interventions when indicated.
  • Take and document vital signs as required by the individual action plan and primary care clinician.
  • Obtain physician signatures for participation and approval at the day habilitation program.
  • Document monthly, annual, and as-needed reviews for people served.
  • Coordinate communication among day program staff, the primary care provider, guardian, and residence when medical status changes.
  • Collect sputum, urine, and stool samples as ordered.
  • Administer medications and treatments as ordered by the primary care provider.
  • Maintain clients’ medical equipment and supplies, including wheelchairs, walkers, Hoyer lifts, and hospital beds.
  • Provide transportation and accompany individuals to medical, dental, or other appointments as needed.
  • Participate in trainings and attend supervision and staff meetings as needed.
  • Follow company, division, and program policies; safe-work practices; universal precautions; exposure-control requirements; accreditation and licensing standards; and applicable regulations.
  • Contribute to the program’s quality-improvement goals and objectives.
  • Use crisis-intervention techniques, access on-call backup when needed, and complete incident documentation.
  • Ensure the maintenance and safety of the physical site.
  • Perform other related duties as required.
