  • A New York State Physical Therapist license is required.
  • At least one year of recent hands-on experience as a Physical Therapist is required.
  • A valid New York State driver's license and reliable transportation are required.
Responsibilities
  • Review patient charts and medical history.
  • Develop personalized treatment plans.
  • Teach therapeutic exercises and stretches to improve patients' conditions.
  • Use hands-on therapy to promote healing.
  • Maintain patient records by tracking goals and progress and adapting treatment plans accordingly.
  • Work closely with physicians, nursing, social services, pastoral care, home health aides, and other members of the patient care team for the assigned caseload.
  • Provide patient and family education for health promotion as directed by the physician's plan of treatment.
  • Provide care according to the required standard.
