- A New York State Physical Therapist license is required.
- At least one year of recent hands-on experience as a Physical Therapist is required.
- A valid New York State driver's license and reliable transportation are required.
- Review patient charts and medical history.
- Develop personalized treatment plans.
- Teach therapeutic exercises and stretches to improve patients' conditions.
- Use hands-on therapy to promote healing.
- Maintain patient records by tracking goals and progress and adapting treatment plans accordingly.
- Work closely with physicians, nursing, social services, pastoral care, home health aides, and other members of the patient care team for the assigned caseload.
- Provide patient and family education for health promotion as directed by the physician's plan of treatment.
- Provide care according to the required standard.
