Synapse Health builds a digital platform that streamlines the medical equipment ordering process for hospice and skilled nursing facilities. Its flagship product, Synapse Equip, centralizes DME ordering, real-time updates, and communication between patients, prescribers, providers, and health plans to speed up orders and cut costs. The system works by letting clients place, track, and manage DME orders in one place, reducing phone calls and delays while providing up-to-date status information. It differentiates itself by focusing specifically on DME for hospice and nursing care, offering end-to-end visibility and service-fee revenue from providers and plans rather than hardware sales. The company's goal is to improve patient and prescriber experience, lower unit costs, and increase customer retention through efficient, technology-driven supply management.
