Requirements
  • A Bachelor’s degree in Pharmacy, including 1,000 hours of internship under a licensed preceptor and 90 days of on-the-job experience at BHG, is required.
  • A pharmacist license in good standing with the state of Michigan is required, along with completion of the required continuing education hours for relicensure every two years.
  • ACLS certification is required for pharmacists regularly scheduled to staff the ED or Adult MICU (one or more shifts per month on average).
  • For oncology-specific duties, PGY-1 training is preferred at a minimum; PGY-2 training in oncology, oncology board certification, or equivalent experience is preferred.
  • Basic word processing skills and the ability to operate hospital programs used in the Pharmacy are required.
  • For anticoagulation stewardship duties, a PharmD, state licensure, and experience in anticoagulation management are required.
  • Strong clinical knowledge and communication skills are required.
  • The work requires sustained alertness and concentration for detailed or repetitive tasks, including full attention to a computer or CRT 40 to 70 percent of the time.
  • The work involves considerable standing or walking, regular lifting of objects weighing 20 pounds or less, assisting with heavier tasks such as moving patients, and some straining or fatiguing positions and motions.
Responsibilities
  • Review, interpret, and fill prescriptions; consult patient drug profiles for allergies, drug interactions, refill status, and pricing; ensure the correct medication and dosage are prepared and dispensed, and that prescriptions are complete and routes of administration are appropriate.
  • Verify orders with physicians as necessary and notify them of potential problems involving dosages, allergies, or contraindications.
  • Provide technical and clinical information and troubleshoot medication-related issues, including drugs, dosages, and contraindications, for physicians, nurses, other healthcare employees, and the community.
  • Record patient prescription data in patient profiles and update profiles with additional orders, allergic reactions, and diagnoses; identify apparent conflicts in patient profiles or drug therapy programs.
  • Prepare or oversee preparation and dispensing of intravenous medications; evaluate prescriptions for dose and schedule accuracy and enter information in patient profiles. Prepare large-volume parenterals, IV piggybacks, total parenteral nutrition, chemotherapeutic and antineoplastic agents, IV push medications, irrigations, patient-controlled analgesics, ophthalmic preparations, and enteral feedings.
  • Assist with overseeing unit-dose and IV additive technicians’ regular work; ensure drugs and solutions prepared by technicians are accurate and prepared under proper conditions; check medication carts for accuracy and release them for delivery to patient care areas.
  • Prepare and check stock drugs for nursing units and other departments; regularly monitor pharmacy and nursing-unit medications for currency and adequate supply.
  • Provide pharmacokinetic evaluation and dosing for patients on specific medications as needed, monitor patient progress, and evaluate and act on laboratory data and patient response.
  • Maintain current knowledge of developments and advances in pharmacy and fulfill continuing education requirements to maintain registration.
  • Maintain the work area and equipment in a clean and orderly condition.
  • Maintain complete and accurate records of dispensed drugs and records of narcotic drugs in accordance with hospital and regulatory requirements.
  • Answer the phone, take medication orders, and provide general information and specific patient data as appropriate.
  • Assist with general department operations, including ordering and receiving medications, resolving equipment problems, restocking medicines, and participating in special projects.
  • Participate in drug utilization reviews and quality assurance activities.
  • For anticoagulation stewardship, develop protocols, monitor therapy, conduct risk assessments, educate patients and staff, and ensure compliance with clinical guidelines.
  • Meet The Joint Commission requirements for anticoagulation through establishing protocols, providing patient education, ensuring regular monitoring, and implementing safety measures.
  • For anticoagulation stewardship, report to the System Clinical Coordinator and work directly with the lead provider to support safe and effective anticoagulant use, with close alignment to the Bronson Anticoagulation Clinics.
  • Provide anticoagulation stewardship across acute care hospitals and Bronson practices.
Desired Qualifications
