Requirements
  • One or more years of experience and/or training in a related function is preferred for employment.
  • A high school diploma or equivalent is required; in some cases, experience may substitute for academic training.
  • A valid driver's license is required, along with adequate insurance when using a personal vehicle, a safe driving record, and successful completion of a driver's screening background check.
  • Applicants must be at least 21 years old and have maintained a valid United States driver's license for at least six months.
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds using proper lifting techniques or a two-person lift, and assist clients with daily living tasks.
  • Ability to operate a computer, calculator, copier, printer, and other office equipment.
  • Ability to communicate with clients, staff, and external representatives and meet specified visual and physical requirements.
  • Ability to assist with routine living activities, including cleaning, meal preparation, vacuuming, shoveling, and grocery shopping.
  • CPR certification is required within two weeks of hire.
  • First Aid certification is required within two weeks of hire.
  • Medication Administration Program certification is required within 150 days of hire.
  • Positive Behavior Supports Tier 1 certification is required within 150 days of hire.
  • Safety Care certification is required within 90 days of hire.
  • Knowledge and use of advocacy techniques, communication styles, empowerment and self-advocacy techniques, participatory planning techniques, physical personal management skills, household management skills, and crisis intervention techniques.
  • Ability to work independently and as part of an interdisciplinary team, adapt communication styles for different individuals, work professionally and confidentially, learn and practice new skills, perform consistently, and communicate effectively orally and in writing.
Responsibilities
  • Assist individuals in the home to become fully integrated into their communities and develop and maintain friendships.
  • Help individuals access community-based support and resources.
  • Plan recreational and social activities that promote social inclusion and emotional well-being.
  • Support individuals in making choices, practicing self-advocacy, and participating in decisions about their supports and lives.
  • Support access to assistive technology and adaptive equipment to augment communication, access communities, and promote independence.
  • Support individuals in securing, maintaining, and growing in work, volunteer, and other community engagement opportunities.
  • Support individuals and their families in maintaining strong partnerships with staff and other interested parties.
  • Provide training and support in rehabilitative and behavioral interventions, fire and safety regulations, appointment scheduling, money management, activities of daily living, grocery shopping, meal planning and cooking, household chores, and use of generic resources.
  • Perform hands-on tasks required to implement support plans.
  • Provide transportation as necessary.
  • Assess individuals' physical and emotional status and report changes to the supervisor.
  • Monitor individual funds as needed.
  • Maintain individual records, collect behavioral and academic data, maintain progress notes, and document incidents, programs, and routine activities in communication logs.
  • Procure, store, administer, document, and dispose of medication consistent with Medication Administration Program requirements, funding-source regulations, and company policy.
  • Communicate with and provide information to program nurses, case managers, and Visiting Nurses Association representatives.
  • Participate in mandated training and staff meetings as directed by the supervisor.
  • Ensure compliance with internal and external regulatory policies, procedures, and standards.
  • Maintain home safety and upkeep, including general housekeeping, snow shoveling, and trash removal.
  • Comply with corporate and divisional policies and procedures, safe work practices, workers' compensation requirements, universal precautions, and exposure-control procedures.
  • Complete communication logs, community membership logs, activity schedules, and other required paperwork.
  • Provide shift coverage in other homes or programs as assigned by the supervisor.
  • Function as a human rights advocate, provide physical intervention and/or First Aid during behavioral or medical emergencies, and serve as a reporter to the Disabled Persons Protection Commission.
  • Assist in developing, implementing, and documenting treatment, including Individual Support Plans, Person-Centered Support and Treatment Plans, and clinical programs.
  • Perform other related duties as required.
