About the job
- Must be able to work the complete recurring Saturday and Sunday, 8:00 PM to 8:00 AM schedule every other weekend.
- Provide care within the LPN scope of practice and in accordance with care plans, physician orders, organizational policies, and applicable requirements.
- Maintain dependable attendance for the complete assigned shift.
- Provide person-centered nursing services to individuals with intellectual and developmental disabilities in accordance with each person's plan of care, physician orders, organizational policies, and applicable requirements.
- Administer medications and treatments safely, and accurately document services in the medication administration record and other required records.
- Monitor each person's health status, identify changes or concerns, take appropriate action, and promptly notify or escalate concerns.
- Complete nursing notes, health assessments, incident documentation, medication reviews, and other required records within established timeframes.
- Coordinate with physicians, pharmacies, hospitals, families, direct support professionals, managers, and other interdisciplinary team members.
- Support continuity of care during admissions, discharges, appointments, transitions, and changes in medication or treatment orders.
- Guide direct support staff on health protocols, medication-related concerns, and person-specific nursing instructions within the LPN scope of practice.
- Follow infection-prevention, confidentiality, safety, and emergency-response procedures; report suspected abuse, neglect, exploitation, or other reportable events as required.
