  • A high school diploma or GED is required.
  • Graduation from a Nursing Assistant Program is required.
  • A current, unrestricted Certified Nursing Assistant or Licensed Nursing Assistant license in Arizona is required.
  • Basic Life Support certification from the American Heart Association is required.
  • If the employee does not have an Arizona Licensed Nursing Assistant license, a Fingerprint Clearance Card application number is required upon hire and a Fingerprint Clearance Card is required within 90 days of hire.
  • Employees must obtain and maintain appropriate clinical competencies for their assigned roles.
  • Candidates for the Discharge Patient Care Technician role must be able to collaborate with interdisciplinary teams and community agencies in patient care and care management.
  • Candidates for the Discharge Patient Care Technician role must be able to use the electronic medical record for documentation and communication.
  • Candidates for the Discharge Patient Care Technician role must know available community resources and understand post-discharge resource planning.

Responsibilities
  • Maintain department cleanliness, including cleanup after treatments.
  • Assist with patient transfers involving weight-bearing restrictions or impaired motor status and transfers to or from wheelchairs.
  • Assist patients with donning and doffing compression garments.
  • Observe skin integrity and report problem areas to supervising clinicians.
  • Assist with removal of catheters and intravenous lines and perform phlebotomy within the scope of practice.
  • Prioritize and solve problems in response to changes in patient status or condition.
  • Collaborate with physicians and other healthcare professionals to support staffing patterns with the skills needed for quality patient care.
  • Assess patient care needs quickly and accurately.
  • Serve as a resource, assist staff with patient care, and assist physicians with order entry into the electronic medical record as needed.
  • Assist with dressing removal as assigned by the supervising clinician.
  • Assist patients with activities of daily living, including toileting.
  • Assist patients with mobility under the direction of the supervising clinician, including therapeutic exercises.
  • Follow safety guidelines for isolation precautions and policies concerning bloodborne pathogens.
  • Assist discharged patients with entering and exiting vehicles.
  • Support and reinforce discharge instructions.
  • Collaborate with interdisciplinary teams and community agencies.
  • Use the electronic medical record for documentation and communication.
  • Assist with securing durable medical equipment as necessary.
  • Answer, evaluate, and prioritize incoming administrative telephone calls; communicate with callers and obtain complete, accurate information to determine their needs.
  • Assist with orientation and training of new Patient Care Technician staff.
  • Work to ensure best practices among colleagues.
  • Maintain unit documents and charts and copy charts as requested.
  • Assist the charge nurse with clerical duties.
  • Document objective patient data and care provided, including reports of abnormal findings to the supervising clinician.
  • Understand and follow unit standards of care, practice standards, administrative policies, and applicable nursing-process guidelines and Arizona nursing laws and regulations.
  • Report safety incidents through the Midas/RDE tool, attend safety training, work safely, and monitor the work environment for safety issues.
  • Comply with applicable state and federal regulations and company policies.
  • Keep required certifications and licenses current when applicable.
  • Complete mandatory company modules and job-specific training within the specified time frame.
