Requirements
  • High school education required
  • Valid Certified Nursing Assistant certification required
  • Must be a graduate of an approved Nurses' Aide Training Program, or be eligible to enter such a program, graduate and pass a state approved competency evaluation within 120 days from the date of employment
  • Must be a minimum of eighteen years of age
Responsibilities
  • Supports the medical/clinical needs of the resident by transporting and timely preparation for rehabilitation therapy, medical tests, and consultation/physician appointment
  • Collects timely and accurate specimens, vital signs, weights and measurements
  • Reports findings to nurse or other appropriate clinicians
  • Contributes to the overall care, concern and comfort of all residents on the wings
  • Assists in the maintenance and enhancement of the residents personal space and environment by making beds, maintaining room temperature, assuring personal preferences of stimulation are offered/supplied i.e.: music, radio, snoezelen, activity box
  • Provides accurate and appropriate feedback, communication and evaluation to resident and family regarding care provided by CNAs
  • Follows standard Precautions Guidelines for Infection Control
  • Reports any communicable or infectious disease to the shift Supervisor
  • Provides personal care to residents with a focus on resident preferences in regard to how and when care is provided for meal assistance, feeding, washing, bathing, mouth care, dressing, hair care, nail care, shaving and bed/chair positioning
  • Follows the plan of care in handheld PDA assigned residents
  • Provides timely and accurate documentation
  • Helps to improve or maintain resident’s level of functioning and health status through proactive and timely assistance with bowel and bladder functions
  • Recording intake and output, ambulation, transfer, range of motion, use of assistive devices/splints
  • Appropriate use of restraints, per policy
  • Provides comfort to resident throughout the day with a focus on resident preferences, including regular repositioning in bed or chair, changes of clothes or bedding when soiled or wet and pain management through key times rounding and answering call bells within standard time
  • Checks and documents proper functioning and use of specialty mattress
  • Assures nurse call bell is always within reach and call bells are answered promptly
  • Observes and reports all unusual changes, occurrences, including physical or emotional status, functioning, behavior; including falls, skin breakdown, pressure wounds, complaints, grievances and clinical needs
  • Reviews and updates Personal Care Records
  • Supports the resident and family during transitions of admission, transfer or discharge including orientation and welcome to unit/room, packing/unpacking of personal belongings
  • Encourages and assists resident in participation of activities and social programs, i.e.: parties, church services, outdoor programs, snoezelen
  • Regularly uses resident activity boxes
  • Offers timely and regular transport to activities
  • Demonstrates consistent reliability/dependability through attendance and punctuality
  • Adheres to Christian Health’s Code of Ethical and Legal Behavior, including completion of specific training and educational programs as deemed necessary for the position
  • Cooperates and contributes to company initiatives as deemed appropriate for increased patient satisfaction
  • Carries out QAPI roles and responsibilities as assigned in an effort to improve processes involved in health care delivery and resident quality of life
  • Follows established policies and procedures in support of CH QAPI efforts to ensure high quality care
Desired Qualifications
  • Six (6) months Nursing Assistant experience preferred
