Requirements
  • High school education is required.
  • A valid Certified Nursing Assistant certification is required.
  • The candidate must be a graduate of an approved Nurses' Aide Training Program, or be eligible to enter such a program, graduate, and pass a state-approved competency evaluation within 120 days of employment.
  • The candidate must be at least eighteen years of age.
  • The candidate must demonstrate compliance with the Christian Health ASPIRE Standards of Performance.
Responsibilities
  • Provide residents with routine nursing care in accordance with established Christian Health policies and procedures in Heritage Manor.
  • Transport residents and prepare them in a timely manner for rehabilitation therapy, medical tests, and physician consultations.
  • Collect timely and accurate specimens, vital signs, weights, and measurements, and report findings to nurses or other appropriate clinicians.
  • Contribute to the care, concern, and comfort of residents and assist the unit as needed.
  • Encourage residents to participate in self-care activities of daily living as part of rehabilitation or maintenance of abilities.
  • Maintain and enhance residents' personal spaces and environments by making beds, maintaining room temperature, offering preferred stimulation, and ensuring equipment, furnishings, and spaces are clean and in good repair.
  • Provide accurate and appropriate feedback, communication, and evaluation to residents and families regarding care provided by Certified Nursing Assistants, and refer clinical or medical questions to nurses or appropriate clinicians.
  • Follow standard precautions and infection-control guidelines, report communicable or infectious diseases, and follow established isolation precautions and procedures.
  • Provide personal care according to resident preferences, including meal assistance, feeding, washing, bathing, mouth care, dressing, hair care, nail care, shaving, and bed or chair positioning.
  • Follow the plan of care for assigned residents using the handheld personal digital assistant and provide timely, accurate documentation.
  • Help improve or maintain residents' functioning and health status through assistance with bowel and bladder functions, recording intake and output, ambulation, transfers, range of motion, assistive devices or splints, and policy-compliant restraint use.
  • Provide comfort through repositioning, changing soiled clothing or bedding, pain-management support during key-time rounding, answering call bells within standard time, checking specialty mattresses, and keeping nurse call bells within reach.
  • Observe and report unusual changes and occurrences involving physical or emotional status, functioning, behavior, falls, skin breakdown, pressure wounds, complaints, grievances, and clinical needs.
  • Review and update Personal Care Records.
  • Support residents and families during admission, transfer, and discharge transitions, including orientation to the unit or room and packing or unpacking personal belongings.
  • Encourage and assist residents to participate in activities and social programs, and provide timely transportation to activities.
  • Complete required training and educational programs under Christian Health's Code of Ethical and Legal Behavior.
  • Cooperate with and contribute to company initiatives intended to increase patient satisfaction.
  • Carry out assigned Quality Assurance and Performance Improvement roles and responsibilities to improve healthcare-delivery processes and resident quality of life.
  • Follow established policies and procedures supporting Christian Health Quality Assurance and Performance Improvement efforts to ensure high-quality care.
Desired Qualifications
  • Six months of Nursing Assistant experience is preferred.
