Requirements
  • Three to five years of experience in health and social care.
  • At least two years of experience as a Care Manager, Field Care Supervisor, Deputy Manager or Team Leader.
  • Experience within live-in care, domiciliary care, home care, community care or complex care.
  • Strong knowledge of Care Quality Commission standards, safeguarding, risk assessments and compliance.
  • Experience completing care assessments and person-centred care planning.
  • Experience supervising and supporting care staff.
  • Strong communication and relationship-building skills.
  • Strong organisational and case management skills.
  • Confidence using digital care systems and Microsoft Office.
  • A full United Kingdom driving licence and access to a personal vehicle.
  • Based within commuting distance of London.
Responsibilities
  • Attend morning meetings with the care management and quality assurance teams to align on action plans.
  • Complete new client assessments and ensure smooth onboarding of new clients.
  • Complete field and virtual visits to assess professional carer wellbeing and performance, including spot checks, supervisions and annual appraisals.
  • Complete field and virtual visits to assess client wellbeing, care plan suitability and environmental risks.
  • Collect quality assurance forms from clients’ homes where required and support their auditing and feedback.
  • Develop and maintain strong relationships with clients and their families, providing ongoing support and assistance as needed.
  • Work with healthcare professionals, including doctors, nurses and social workers, to ensure clients receive appropriate medical care and support.
  • Ensure compliance with relevant legislation and regulations, including health and safety and safeguarding requirements.
  • Create outcome-based care plans according to the client quality assurance schedule and on an ad hoc basis, and update them as required.
  • Manage the field visit schedule two weeks in advance.
  • Report field data to the care management team and against company key performance indicators.
  • Handle complaints and disciplinary processes in accordance with company policies and procedures.
  • Cover the out-of-hours service on a rotational basis every five to six weeks.
