Requirements
  • A Master's degree in social work from a Council on Social Work Education-accredited school is required.
  • A Licensed Master Social Worker credential is required, or must be obtained within six months of employment.
  • Bilingual English and Spanish proficiency is required.
  • At least two years of experience in a health care setting, HIV/AIDS program, alcohol or substance use program, and/or mental health program is required.
  • Proficient computer skills and computer operations are required.
  • A valid driver's license is required.
  • Access to a personal car for work-related travel is required.

  • The ability to plan, organize, develop, and implement goals, objectives, policies, and procedures necessary for quality care is required.

Responsibilities
  • Provide client-centered, strengths-based care management services, including outreach, engagement, assessment, care plan development and implementation, coordination, and monitoring of assigned clients.
  • Coordinate and advocate for medical, substance use, mental health, and social support services to promote optimal health outcomes and reduce emergency department use and inpatient hospital stays.
  • Conduct active and progressive outreach and engagement activities, obtain program-specific consent, and support clients' ongoing participation in appropriate programs.
  • Conduct comprehensive intake assessments within 30 days of initial consent or agreement to participate, including the program-specific assessment packet, Patient Health Questionnaire-9, and Health and Recovery Plan eligibility assessment using the Uniform Assessment System.
  • Develop and implement individualized care plans within 30 days of client assignment, and review and update them regularly based on client needs and preferences.
  • Protect clients' confidential health care and personal information, maintain current consent forms, limit information disclosures to authorized parties, and safeguard electronic devices and passwords.
  • Coordinate all required client services and use motivational interviewing, role playing, modeling, and teach-back interventions to support independence and self-management.
  • Accompany clients to service-provider appointments to support access, understanding, and required follow-up.
  • Coordinate the overall care of assigned clients, conduct ongoing case conferences with clients and care teams, share and discuss care-plan information appropriately, and complete case-conferencing documentation.
  • Provide clinical casework interventions to the assigned caseload.
  • Document all client interventions in the appropriate program platform using the Problem/Intervention/Follow-up format, track interventions in client contact logs, and submit required monitoring data on schedule.
  • Travel independently between multiple off-site locations, including community meetings, provider sites, clients' homes, and other locations needed to meet care-plan goals.
  • Provide culturally and linguistically appropriate customer service to internal and external customers, clients, and patients.
  • Participate in the department's quality improvement and quality assurance efforts, including the Quality Improvement Committee.

St. John's Riverside Hospital St. John's Riverside Hospital, a leading community-based healthcare provider in Yonkers, New York, recently experienced a data breach that potentially exposed personally identifiable information (PII) and protected health information (PHI) of at least 2,238 individuals across the U.S. According to a disclosure filed with the U.S. Department of Health and Human Services on Nov. 14, 2025, the breach originated from unauthorized access to a limited number of employee email accounts. The incident was discovered in September 2025, when the hospital detected suspicious activity involving the distribution of phishing emails and an attempt to reroute payment funds. The compromised email accounts contained a range of sensitive information. For some individuals, the exposed data included personally identifiable information (PII) such as name, date of birth, Social Security number, driver's license or state identification number and financial account number. For others, protected health information (PHI) was involved, including health insurance details, medical condition information, treatment provider name, medical record number, treatment cost information and diagnosis or treatment information. St. John's Riverside Hospital's response. Upon discovering the breach, St. John's Riverside Hospital immediately took steps to secure its systems. The hospital changed passwords, revoked session tokens, reset multifactor authentication and engaged data security and privacy professionals to investigate the incident. The unauthorized activity was contained and remediated, and the hospital conducted a thorough analysis to identify affected individuals.
