Requirements
  • Current Registered Nurse licensure in good standing in the state or states of practice, with one year of experience under license.
  • Ability to work independently with phone, email, written communication, meetings with healthcare professionals, and hospice rules and regulations.
  • Time-management skills to schedule visits, complete documentation, attend required meetings, and prioritize and triage multiple urgent concerns.
  • Ability to learn an electronic medical records system.
  • Willingness to drive across the service area with reliable transportation, a valid driver's license, and auto insurance.
Responsibilities
  • Provide care and services to a group of hospice patients in private homes, assisted living facilities, and hospitals.
  • Complete hospice admissions and patient visits as needed.
  • Answer calls from patients, families, and customers.
  • Conduct nursing visits for symptom management, deaths, and admissions as needed.
  • Advocate for hospice services and Suncrest Hospice.
  • Collaborate daily with nurses, social workers, chaplains, volunteers, and certified nursing assistant leadership.
  • Work one-to-one with patients while receiving support from the patient care team.
