Responsibilities
  • Provide psychosocial services to in-center and home dialysis patients using social work theory and accepted social work practice methods.
  • Collaborate with the interdisciplinary healthcare team to promote patient adjustment, rehabilitation, quality of life, and comprehensive care.
  • Assess patients' psychosocial status, strengths, and needs as part of comprehensive patient assessments.
  • Participate in care planning with patients and the healthcare team to identify interventions supporting rehabilitation and treatment goals.
  • Use patient education programs, social work interventions, and quality-of-life measurement instruments to address barriers and treatment goals.
  • Monitor and intervene to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life.
  • Provide supportive counseling within the scope of clinical training and state licensure.
  • Provide education and goal-directed counseling to patients seeking transplantation.
  • Refer patients to community resources, including home health services and vocational rehabilitation.
  • Maintain knowledge of local vocational and educational rehabilitation programs and assist patients with referrals and access.
  • Assess patient awareness of advance directives and assist with forms, information, and related discussions.
  • Participate with the physician and nurse in discussions of patient do-not-resuscitate status to support informed decisions.
  • Follow social work policies, measurement policies, and documentation requirements.
  • Document patient and family interactions and interventions.
  • Provide psychosocial support and social work interventions addressing non-adherence, quality outcomes, and quality-of-life concerns.
  • Participate in monthly quality review meetings and report adherence and quality-of-life indicators.
  • Assess patient knowledge of kidney disease and provide education tailored to learning styles, communication barriers, and needs.
  • Provide information about available treatment modalities with the interdisciplinary team.
  • Facilitate transplant referrals and collaborate on transplant waitlist management.
  • Educate patients and families about psychosocial issues related to end-stage renal disease and available support services.
  • Review patient rights, responsibilities, grievance information, and facility policies with patients or representatives.
  • Collaborate with the team on quality assessment and improvement activities.
  • Review admission-related rights, grievance, nondiscrimination, and do-not-resuscitate materials and address immediate concerns.
  • Support referral, admission, scheduling, continuity-of-care, and patient-adjustment needs.
  • Interview patients requesting discharge to identify root causes and share concerns with operational leadership.
  • Liaise with insurance coordinators regarding insurance issues and changes affecting coverage.
  • Educate patients about dialysis payment sources and assistance programs.
  • Refer billing questions and treatment-related bill concerns to the patient billing solutions department.
  • Assist with interviewing and hiring decisions for new personnel when requested by social work management.
  • Maintain updated patient resource lists with administrative support staff.
  • Provide psychosocial training to staff as needed.
  • Participate in weekly team huddles and discuss urgent patient issues with staff.
  • Follow caseload guidelines based on state regulatory requirements.
  • Travel between facilities when required.
