Requirements
  • Graduation from an accredited registered nursing school is required.
  • A current Georgia Registered Nurse license, Nursing Compact license, or multistate license is required.
  • Two years of previous clinical experience is required.
Responsibilities
  • Work with neonate, adolescent, adult, and geriatric patients, families, medical staff, and community agencies to determine the appropriate care setting and appropriateness of hospitalization.
  • Assess post-hospital care needs.
  • Provide support to address social drivers of health.
Desired Qualifications
  • A Bachelor of Science in Nursing is preferred.
  • Three years of experience in case management, social services, or a related field is preferred.
  • A Certified Case Manager credential is preferred.
  • Ability to interact with patients, families, physicians, and hospital staff.
  • Excellent oral and written communication skills.
  • Ability to type accurately at an approximate rate of 30 words per minute.
