- Graduation from an accredited registered nursing school is required.
- A current Georgia Registered Nurse license, Nursing Compact license, or multistate license is required.
- Two years of previous clinical experience is required.
- Work with neonate, adolescent, adult, and geriatric patients, families, medical staff, and community agencies to determine the appropriate care setting and appropriateness of hospitalization.
- Assess post-hospital care needs.
- Provide support to address social drivers of health.
- A Bachelor of Science in Nursing is preferred.
- Three years of experience in case management, social services, or a related field is preferred.
- A Certified Case Manager credential is preferred.
- Ability to interact with patients, families, physicians, and hospital staff.
- Excellent oral and written communication skills.
- Ability to type accurately at an approximate rate of 30 words per minute.
