Requirements
- Graduate of a Commission on Collegiate Nursing Education-accredited nursing program with a Master’s Degree in Nursing.
- National certification by one of the recognized Advanced Practice Registered Nurse certification centers is required.
- Licensure or eligibility for licensure as a professional Registered Nurse and Advanced Practice Registered Nurse in Pennsylvania is required.
- American Heart Association Basic Life Support certification is required, with additional Advanced Life Support training at the discretion of the department.
- A Bachelor’s Degree or Master’s Degree as a Medical/Surgical Physician Assistant is required.
- Graduation from an Accreditation Review Commission on Education for the Physician Assistant-approved Physician Assistant training program is required.
- Certification by the National Commission on Certification of Physician Assistants is required.
- Licensure or eligibility for licensure by the Physician Assistant Board of Medicine is required.
- Proficiency with computers is required.
- The position requires the ability to lift, move, stoop, reach, position patients, and carry patients, materials, and equipment weighing 40 or more pounds.
- The position requires working closely with others, protracted or irregular hours, biohazards, infectious diseases, deceased persons, hands in water, and electrical hazards associated with patient care equipment.
Responsibilities
- Manage comprehensive and individualized care for primary care patients and their families in the ambulatory setting.
- Conduct consultations, pre-procedure evaluations, acute visits, and wellness visits.
- Complete integrated assessments.
- Order additional tests or consultations in collaboration with primary care faculty or according to established clinical guidelines and protocols.
- Perform diagnostic tests and communicate their results according to established clinical guidelines and scope of practice.
- Consult and communicate with clinic and hospital staff regarding abnormal findings in patient evaluations.
- Perform comprehensive clinical evaluations and document findings in the medical record.
- Facilitate patient follow-up by coordinating appropriate services and tests for clinic follow-up appointments.
- Develop individualized plans of care that include teaching, follow-up planning, and referrals to other healthcare team members.
- Care for patients across the age continuum within the scope of educational experience and certification.
- Identify unstable or complex primary care situations and facilitate appropriate intervention in the clinic or hospital setting.
- Recognize emergency situations and initiate effective emergency care.
- Evaluate primary care patients and coordinate follow-up procedures and tests in the ambulatory setting.
- Assist with the collection of outcome measures.
- Apply critical thinking and diagnostic reasoning in clinical decision-making to identify, evaluate, and manage individual patient and family needs.
- Use equipment safely.
- Base decisions and actions on scientific principles, standards of care, and research findings.
- Provide education to nursing staff, hospital staff, students, patients, families, and the community using appropriate teaching and learning strategies.
- Promote an environment that facilitates learning by responding to patient and internal preferences, priorities, and readiness to learn.
- Maintain and update professional knowledge throughout the year.
- Complete all identified clinical competencies.
- Serve as a resource for staff caring for primary care patients.
- Provide educational programs to professional and paraprofessional personnel in the area of expertise.
- Participate in community programs and distribute literature as appropriate.
- Document plans of care, interventions, and evaluations clearly and concisely for every patient visit in the medical record according to approved policies and documentation standards.
- Collaborate with leaders and staff to achieve patient-outcome improvements in care.
- Provide leadership in developing and implementing changes in primary care practice that positively affect patient outcomes.
- Work collaboratively with medical and nursing staff to solve problems and provide professional support.
- Participate in and initiate interdisciplinary approaches to patient care, program development, and education, and serve as a professional role model.
- Promote and practice innovation in the expanded role of care delivery to primary care patients and families in inpatient and ambulatory settings.
- Participate in and provide leadership for committees and task forces.
- Document hours and submit billing reports as appropriate.
- Review and interpret primary care data, including patient registries, clinical and financial reports, length of stay, access to care, and cost-per-case reports, and recommend or implement changes as necessary.
- Participate in clinical trials and research studies with the Principal Investigator.
- Assist primary care faculty, administrators, and directors with collecting outcome measures.
- Perform other assigned duties and projects.
Desired Qualifications
- Primary care experience is preferred.
- Advanced Practice Registered Nurses certified as Adult-Gerontology Acute Care Nurse Practitioners or Family Nurse Practitioners are considered.
- A Master’s Degree as a Physician Assistant is preferred.
