Requirements
  • At least one year of experience as a registered nurse in acute care hospital, oncology, long-term care, hospice, or home health settings is required; school internships and clinical hours may be considered in individual cases.
  • A current license to practice nursing in the state of requested employment is required.
  • Current CPR certification is required.
  • Current automobile insurance and a valid driver’s license are required.
  • Knowledge of terminally ill patients and their families, hospice care, the roles of disciplines providing hospice services, community resources, general nursing practice, managed care principles, and regulatory guidelines including Medicare, Medicaid, and ACHC is required.
  • Excellent patient assessment skills, documentation and program management knowledge, and effective oral and written communication skills are required.
  • The employee must maintain transportation for work and be able to operate nursing assessment tools, computers, facsimile equipment, a copier, and a cell phone or beeper.
  • The employee must be able to work in patients’ homes or institutional settings and manage potential exposure to infections, communicable diseases, odors, blood, excreta, and hazardous materials.
Responsibilities
  • Serve as the primary point of contact for patients and families throughout the hospice journey.
  • Deliver and document skilled nursing care according to individualized plans of care in patients’ homes, assisted living facilities, or other locations where patients live.
  • Perform initial, ongoing, comprehensive, and reassessment-based patient screenings, including in-person and telephonic registered nurse assessments.
  • Monitor patient condition, identify changes or decline, and communicate findings to physicians, nurse practitioners, primary care providers, hospice physicians, and the care team.
  • Develop and update individualized plans of care, including patient visit frequency and risk identification.
  • Deliver skilled nursing interventions and symptom management aligned with goals of care.
  • Coordinate care with interdisciplinary teams, including physicians, licensed practical nurses, certified nursing assistants, social workers, chaplains, and other disciplines.
  • Provide patients, families, and caregivers with education about disease processes, self-care techniques, end-of-life care, medications, nutrition, dietary needs, hospice philosophy and services, symptom and pain management, and end-of-life processes and expectations.
  • Teach caregivers to provide safe and confident care and offer emotional, physical, and spiritual support, comfort, and bereavement guidance.
  • Recognize and support home health, palliative, and hospice referrals and transitions in levels of care as appropriate.
  • Maintain timely and accurate clinical records, including screening and assessment findings, physician orders, progress notes, and care plans.
  • Follow infection-control, regulatory, compliance, and policy standards.
  • Maintain effective working relationships with the interdisciplinary group and involve the team when patient care plans require participation.
  • Participate in quality-improvement, scheduling, on-call, and patient-support activities, conducting on-call services in a clinically competent and responsive manner.
