Requirements
  • A high school diploma or GED is required.
  • A valid driver's license is required, including the ability to drive a large wheelchair-accessible vehicle or 15-passenger van.
  • At least one year of experience in human services is required.
  • Experience in medical care and interaction with medical professionals is required.
  • The employee must successfully complete AMAP certification training and maintain valid certification throughout employment.
  • SCIP-R, cardiopulmonary resuscitation, and first aid are required.
  • The employee must demonstrate oral and written communication skills, willingness to learn computer systems, teamwork, professional conduct, and respect for residents served.
Responsibilities
  • Provide and ensure a safe, clean, normalized, home-like environment and inform the Program Manager of physical plant needs.
  • Inform the Program Manager or Assistant Program Manager promptly of serious incidents, rights violations, deficiencies, abuse, safety issues, and other facility problems.
  • Develop residents' Activities of Daily Living and Independent Living Skills.
  • Maintain knowledge of each resident's medical, psychological, cognitive, and social needs and ensure programming aligns with treatment goals.
  • Maintain and submit accurate records and reports, including daily goal data, monthly progress notes, summaries, medical consultation reports, incident reports, and residence logbook entries.
  • Maintain and check medication administration records, order medications according to doctors' orders, schedule medication appointments, review medical records, and communicate appointment follow-up needs with nursing and management.
  • Participate in interdisciplinary treatment meetings and scheduled resident reviews.
  • Provide habilitation services according to each client's service plans, including the CFA, Individual Service Plan, and Habilitation Plan.
  • Transport or escort individuals to programs, community inclusion activities, medical appointments, home visits, and other activities as needed.
  • Assist clients with activities of daily living, domestic living, recreation, socialization, nutrition, and feeding as needed.
  • Organize, assist with, supervise, and prepare resident meals when necessary.
  • Assume responsibility for assigned agency or resident funds used for resident or program purchases.
  • Complete Staff Information Entry Forms when escorting individuals to medical appointments.
  • Read the residence logbook daily and communicate pertinent resident and program issues verbally and in writing to the Program Nurse and supervisors.
  • Assist the Housekeeper/Cook with household cleaning and facility maintenance and perform housekeeping duties as assigned.
  • Maintain attendance according to assigned shifts and management direction.
  • Successfully participate in medication certification courses and demonstrate continued proficiency in medication administration.
  • Perform related duties and other duties as assigned by the Program Manager.
Desired Qualifications
