Requirements
  • A high school diploma or equivalent.
  • Three years of full-time, paid experience providing support and assistance to staff in the development and/or operation of programmatic activities for school-age children, identifying the social service needs of students and their families in a large urban setting, and/or developing and maintaining databases.
  • Knowledge of Microsoft Word and Excel.
  • Knowledge of the social, economic, and health problems common in urban communities.
  • Knowledge of community resources available to meet social, economic, and health needs.
  • Ability to identify and resolve problems related to programmatic activities.
  • Ability to use and operate a personal computer.
  • Ability to communicate effectively, both orally and in writing.
  • Ability to establish and maintain effective working relationships.
  • Knowledge of the principles, practices, and techniques of case management.
  • Knowledge of the practices and techniques of providing students and families with optimal service.
  • Knowledge of school-based programs.
  • Knowledge of personal computer software applications used to create reports and documents and design spreadsheets.
  • Skill in organization with attention to detail.
  • Ability to provide resources available for students and families.
  • Ability to ensure case records meet documentation standards.

Responsibilities
  • Assist in the coordination, implementation, and operation of one or more program initiatives in the office and participate in the development of programmatic activities.
  • Keep abreast of reporting requirements for the programs and work with appropriate staff to ensure deadlines are met on time.
  • Process paperwork and documentation related to program activities.
  • Collect, collate, and consolidate paperwork and data from programs or participating organizations.
  • Create and maintain databases of information related to contracted agencies or programs, enter new information, edit existing information, extract information to create spreadsheets, and generate reports.
  • Disseminate information to agency partners, schools, and appropriate offices.
  • Keep superiors and staff apprised of program activities and developments and provide up-to-date information.
  • Assist agencies, superiors, or staff with completing program documents, ensuring documents are accurate and submitted in a timely manner.
  • Manage a caseload of up to 15 youth at risk for violence and gang involvement.
  • Provide weekly contact, support, and mentorship to youth on the caseload.
  • Link youth and families to opportunities, services, and assistance programs in the community.
  • Attend weekly team and multidisciplinary team meetings with project partners.
  • Document all activities promptly using designated case management software to maintain accountability and support data reporting requirements.
  • Work effectively with high-risk youth populations.
