About the job
- An active, unrestricted Nurse Practitioner license in Maine is required.
- At least 2 years of clinical experience in neurology, memory care, and/or dementia care settings is required.
- Strong clinical judgment and clinical documentation skills are required.
- The ability to manage a full patient panel independently while collaborating with physicians is required.
- The ability to communicate effectively with patients facing cognitive decline and their caregivers is required.
- Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth.
- Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians, including prescribing therapies.
- Partner with neurologists and the broader care team to ensure continuity of care.
- Educate patients and families on diagnoses, treatment plans, and disease progression.
- Maintain accurate and timely clinical documentation in the electronic medical record consistent with regulatory requirements.
- Certification in geriatrics, adult-gerontology, or family practice is preferred.
