About the job
- A minimum of one year of Registered Nurse experience, preferably in an acute, skilled nursing facility, home health, or hospice setting.
- An active California Registered Nursing license.
- An Associate degree, Bachelor of Science in Nursing, or Master of Science in Nursing degree.
- Reliable transportation.
- A current cardiopulmonary resuscitation or basic life support card.
- Monitor each patient's overall health.
- Develop individualized plans of care in partnership with the patient's primary care physician.
- Function independently as a case manager for assigned patients.
- Deliver quality healthcare to patients in their homes.
- Provide customized care and patient education about disease management.
- Provide simple to complex wound therapy, post-surgical care management, and ostomy care management.
- Plan patient visits and schedules around patient-care needs and other commitments.
- Coordinate medication reconciliation, durable medical equipment orders, physician signatures, coding, and laboratory follow-up with the internal operational support team.
