About the job
- Current licensure by the California Board of Registered Nursing or Vocational Nursing.
- Current CPR certification.
- Knowledge of Centers for Medicare & Medicaid Services regulations and long-term care standards.
- Ability to work independently and as part of an interdisciplinary team.
- All employees must pass required background checks, including a fitness-for-duty physical, fingerprinting, and employment-reference checks.
- Coordinate and complete clinical admissions for new residents in a timely and accurate manner.
- Perform comprehensive nursing assessments upon admission.
- Review hospital records, discharge summaries, medication lists, and physician orders.
- Initiate and document care plans based on resident needs.
- Communicate with physicians, interdisciplinary team members, residents, and families.
- Ensure admission documentation meets regulatory and facility standards.
- Perform wound assessments, treatments, and dressing changes according to physician orders.
- Ensure compliance with infection-control, safety, and quality-assurance standards.
- Maintain accurate and timely documentation in the electronic medical record.
- Participate in care-plan meetings and interdisciplinary rounds as required.
- Assist with audits, surveys, and regulatory-compliance activities.
- Support quality-improvement initiatives related to admissions and wound care.
- Two years of long-term-care experience with prior charge-nurse experience.
- Experience with wound care and admissions.
- Strong assessment, organizational, and communication skills.
