Pharmacy First Health builds clinical infrastructure to bring primary care and chronic disease management into rural community pharmacies. It does this by turning independent pharmacies into integrated care hubs that offer on-site primary care, pharmacy services, and disease management, working with health plans to recognize neighborhood pharmacies as care locations. A digital platform lets pharmacists, clinicians, and care teams coordinate under state-approved Collaborative Practice Agreements, handling secure documentation, scheduling, billing for clinical encounters, and sharing medical records among authorized professionals; the platform does not provide automated medical advice, and licensed clinicians make all medical decisions. The goal is to improve access to primary care in underserved areas by making local pharmacies the entry point for care.
