Requirements
  • The candidate must have a high school diploma or General Educational Development equivalency.
  • The candidate must have a Home Health Aide certification meeting state and Open Systems Health standards.
  • The candidate must provide evidence of tuberculosis screening meeting Centers for Disease Control and Prevention guidelines.
  • The candidate must provide evidence of competency meeting Open Systems Health and state requirements.
  • The candidate must have a reliable form of transportation.
  • The candidate must pass the Open Systems Health competency examination with a score of 80% or greater.
Responsibilities
  • Ensure quality and safe delivery of home care services in alignment with Open Systems Health, federal and state policies and procedures, Occupational Safety and Health Administration requirements, and Centers for Disease Control and Prevention recommendations.
  • Participate in developing and implementing the consumer plan of care according to Open Systems Health policies and procedures.
  • Establish and maintain constructive and cooperative interpersonal relationships with consumers.
  • Assist consumers with personal care services according to their care plans, including bathing, grooming, toileting, urinary drainage bag changes, transfers, ambulation, exercise, medication self-administration, vital-sign recording, observation and reporting, meal preparation, infection control, light housekeeping, simple dressing changes, support for skilled therapy services, and routine care of prosthetic and orthotic devices.
  • Use universal precautions while providing care.
  • Report unusual incidents or changes in consumer behavior to the case manager.
  • Communicate relevant consumer information confidentially and in a timely manner with Open Systems Health.
  • Accurately document services provided according to the consumer’s plan of care.
  • Participate in annual training.
  • Organize, plan, and prioritize work.
  • Comply with federal, state, and local laws.
  • Comply with all Open Systems Health policies and procedures.
