Requirements
  • A high school diploma or GED is required.
Responsibilities
  • Assist each consumer with skill development in independent living, adaptive behaviors, socialization, recreation, and activities of daily living.
  • Supervise consumers in the community, use experiences to reinforce skills taught in group sessions, and ensure their safety.
  • Lead group sessions with consumers and facilitate discussions that provide skills for use in the community.
  • Provide annual and semi-annual progress reports regarding valued outcomes.
  • Assess the needs of lower-functioning and/or non-verbal consumers through observations and evaluations, and develop and address their valued outcomes.
  • Provide input and ensure compliance with valued outcomes in the Individual Service Plan and Day Habilitation Plan.
  • Provide weekly documentation of outcomes being worked toward with each individual in accordance with the Day Habilitation Plan.
Desired Qualifications
  • An associate degree is preferred.
  • One year of experience providing direct services to people with developmental disabilities is preferred.
  • AMAP, SCIP-R, CPR, and First Aid certifications are preferred.
