Requirements
  • Possess a Bachelor of Science in Nursing upon hire for new hires from March 31, 2014 onward.
  • Have at least 3 years of experience in an acute care hospital setting.
  • Hold a Registered Nurse license in good standing with the State of Michigan.
  • Obtain Basic Life Support certification by completion of core orientation.
  • Use word processing, spreadsheet, keyboard, presentation, and other software relevant to the job.
  • Assess, prioritize, plan, organize, monitor, and evaluate patient needs and skill level.
  • Correctly prioritize multiple demands in a stressful situation.
  • Anticipate patient needs and work to resolve them quickly.
  • Work independently and be self-motivated.
  • Use effective negotiation and conflict-resolution skills.
  • Perform work involving sustained mental and visual attention, including operating a computer or cathode-ray tube 40–70% of the time, and regularly walk, stand, stoop, bend, sit, and lift light objects.
  • Work with the multidisciplinary team and providers to create standard care plans for patients, and review care plans or behavioral contracts with patients as needed.
  • Assess uninsured and underinsured patients' eligibility for federal, state, and community programs that assist with funding medical care; help patients process paperwork and collaborate with Patient Financial Counselors.
  • Function as a liaison to community programs, participate in community meetings, and maintain contacts to identify resources for patient needs.
Responsibilities
  • Move patients from admission through discharge without disrupting care by assessing, planning, implementing, coordinating, monitoring, and evaluating assigned caseloads.
  • Ensure care is appropriate to patient needs and hospital capabilities, and serve as an advanced clinical resource to patients, families, and staff.
  • Collaborate with the interdisciplinary team to provide continuous, comprehensive, cost-effective care, and monitor outcomes for continuous improvement.
  • Demonstrate competencies specific to the population served when providing direct patient care.
  • Assess and identify patients at risk for post-hospitalization care and services; interview patients or families and review relevant health records and psychosocial care needs when indicated.
  • Initiate and facilitate ongoing review and revision of patient transition care plans with care coordination team members.
  • Manage and monitor patient progress and document according to procedure.
  • Conduct ongoing patient assessments and maintain contact during care; round daily on assigned patients.
  • Identify readmissions, their causes, and interventions to prevent further readmissions, and communicate the plan to the multidisciplinary team.
  • Work cooperatively with the healthcare team and ensure smooth transitions of care between services.
  • Drive multidisciplinary team rounds.
  • Document clear and specific transition plans that reflect the patient's level-of-care needs and choices.
  • Implement transition plans that move patients along the care continuum; work with the community to identify and allocate post-discharge needs.
  • Evaluate patient needs for hospital and extended-care resources, including medical social work, pastoral care, rehabilitation, long-term care, home health care, and community resources, and make referrals when appropriate.
  • Liaise among patients, physicians, ancillary services, and community services throughout the patient experience, from diagnosis through post-discharge, to support effective healthcare management and transitional services.
  • Develop, implement, coordinate, and communicate plans of care from the acute phase through transition out of acute care.
  • Build and maintain collegial relationships with physicians, the nursing team, and leaders to support quality care.
  • Coordinate care using Pathways or Plans of Care and contribute to their ongoing development and revision.
  • Participate actively in assigned groups and committees.
  • Ensure appropriate use of community and outpatient resources to support post-discharge care needs.
  • Manage and coordinate discharge plans appropriate to care needs and length of stay, including quality, clinical, and financial outcomes; negotiate, procure, and coordinate patient and family services and resources, and intervene at key points.
  • Evaluate case management outcomes, including length of stay, readmission reports, patient satisfaction, and financial variances; report pertinent variances and translate outcomes into healthcare reimbursement principles.
  • Track and trend outlier length-of-stay data to reduce outlier stays.
  • Coordinate and oversee hospital operations, providing clinical and administrative direction in the absence of Unit Leaders.
  • Resolve crises and conflicts, assess and intervene in problems requiring immediate attention, and inform department managers and directors of problems and resolutions.
  • Investigate complaints and problems raised by patients, visitors, physicians, and staff, and implement appropriate actions and follow-up.
  • Serve as the initial contact for atypical events, conduct initial event assessments, initiate appropriate action to stabilize patients, and notify risk management, CSI, and department managers or directors.
  • Assess safety concerns and take appropriate action.
  • Coordinate admissions, transfers, and patient placement.
  • Monitor hospital capacity and work with the Staffing Office to ensure appropriate staff placement.
  • Conduct regular rounds of hospital departments and units.
  • Maintain utilization review and monitor infection-control data.
  • Lead Safety Check-In meetings on weekends and holidays.
  • Answer Employee Incident Hot Line calls after hours, on weekends, and holidays; provide initial triage, direct employees to the emergency room when appropriate, document calls on the Employee Injury and Illness Incident Report, and collaborate with the emergency-room physician to initiate prophylactic treatment after communicable-disease exposure.
  • Participate in After-Care duties when required.
