Requirements
  • Be a registered nurse, licensed practical nurse, or licensed vocational nurse from an accredited nursing program and be licensed to practice as required by state regulations.
  • Hold a valid driver's license.
  • Be CPR certified and have knowledge of basic first aid.
  • Have experience in direct patient care and assessing or observing clinical conditions.
  • Have experience recruiting, training, and managing people in the healthcare field.
  • Have 3-5 years of nursing experience in assisted living or a related industry.
  • Complete all state-required training to maintain licensure or certification.
  • Be able to work flexible hours due to the demands of the position.
  • Be able to drive for work responsibilities using a company-provided or personal vehicle.
  • Have computer proficiency in Microsoft Office and other technologies.
  • Be able to solve problems using judgment and a realistic understanding of the issues, including when dealing with emotional topics.
  • Be able to define realistic, specific goals and objectives and identify the tasks and deliverables needed to meet them.
  • Be able to communicate information clearly and accurately through spoken communication, including under stressful or demanding conditions.
  • Be able to write reports and business correspondence.
  • Build and maintain productive, work-focused relationships with team members, residents, families, corporate, regional, and divisional resources, and the community.
  • Be able to build high morale and group commitment to goals and objectives.
  • Be able to serve as a punctual, neat, and clean role model for team members.
  • Be able to work varied schedules, including weekends, evenings, and holidays.
Responsibilities
  • Participate in pre-admission screening of prospective residents and ensure required documentation is completed before or upon admission, including nursing assessments, service plans, and assigned forms.
  • Update resident assessments as required by community policy and applicable nursing regulations.
  • Monitor implementation of policies and procedures for compliance with corporate and external regulatory guidelines.
  • Monitor efficient and effective use of Wellness department resources within the approved budget and team-member levels.
  • Ensure team members follow safety regulations covering fire protection, infection control, bloodborne pathogens, hazardous waste, and quality assurance.
  • Supervise timely and accurate completion of documentation required for monthly billing of resident services, ancillary charges, and changes in resident care.
  • Review and follow up on health- or injury-related incident reports and develop or monitor corrective action plans.
  • Report emergency situations, reportable incidents, team-member issues, and observable changes in resident behavior or health status to the Executive Director.
  • Supervise Health and Wellness team members through training, counseling or corrective action, motivation, and performance appraisals.
  • Participate in regulatory inspections and surveys, identify trends, and assist with corrective action plans and troubleshooting resolutions.
  • Oversee and assist with recruitment and selection of care team members.
  • Conduct care plan conferences with residents and responsible family members.
  • Communicate the community's care and services philosophy to current and prospective residents.
  • Address resident, family, and team-member concerns and complaints professionally and compassionately.
  • Coordinate or oversee scheduling and completion of in-house ancillary medical services, including podiatry, physician, dental, psychiatric, laboratory, X-ray, and ambulance services.
  • Coordinate resident care with outside healthcare practitioners, consultants, physicians, providers, and other external providers.
  • Assist with training and education.
  • Perform on-site evaluations of residents admitted to alternate care environments and maintain contact with resident families to support return to the community.
  • Assess health, oversee medication administration, oversee functional and psychosocial status, initiate individualized service plans, manage resident care and services, evaluate effectiveness, and maximize residents' ability to remain in their environment as permitted by the Nurse Practice Act.
  • Evaluate residents, document changes in condition, and notify the Executive Director, resident's physician, and legally responsible party or family of residents' conditions and reactions.
  • Communicate with providers about residents' status and condition changes.
  • Evaluate resident transfer and mobility needs and involve therapy services as indicated to help maintain a safe environment and reduce workers' compensation claims.
  • Perform rotating on-call duties with other Wellness department managers.
  • Perform other duties assigned by the Director of Health and Wellness.
