Requirements
  • A Doctorate Pharm.D. degree from an accredited School of Pharmacy is required.
  • Two years of hospital or ambulatory care experience, or completion of a residency program in a hospital or ambulatory infusion setting, is required.
  • Organization and time-management skills are required.
  • Analytical, problem-solving, planning, and evaluation skills are required.
  • The ability to work effectively with physicians, staff, and patients is required.
  • The ability to function independently and as a member of a team is required.
  • A current California pharmacist license from the California State Board of Pharmacy is required.
  • Basic Life Support Healthcare Provider certification from the American Heart Association is required.
  • Fire Life Safety Training from the Los Angeles City Fire Department is required for positions within Los Angeles City; it must be obtained within 30 days of hire if not held upon hire and maintained through renewal before expiration.
  • Advanced Cardiovascular Life Support Healthcare Provider certification from the American Heart Association is required, except for certain off-site clinics.
Responsibilities
  • The pharmacist provides comprehensive pharmaceutical care through integrated drug distribution, clinical services, and education.
  • The pharmacist accurately and timely verifies medication orders through Cerner.
  • The pharmacist evaluates patient profiles for medication-order appropriateness, including dosage based on patient parameters, duplicate therapy, side-effect profile, cost-effectiveness, and accuracy.
  • The pharmacist verifies all medications dispensed accurately and efficiently.
  • The pharmacist follows investigational drug procedures.
  • The pharmacist oversees daily pharmacy operations, including drug distribution activities and compliance with sterile-compounding regulations.
  • The pharmacist oversees narcotic dispensing, reconciliation, and discrepancy resolution.
  • The pharmacist ensures pharmacy-policy compliance with federal, state, and local laws and regulations governing drugs and controlled substances.
  • The pharmacist recommends and facilitates pharmacy workflow and operational improvements.
  • The pharmacist supervises and coordinates pharmacy-technician responsibilities.
  • The pharmacist mentors and trains pharmacy technicians and pharmacy interns.
  • The pharmacist maintains working relationships and communicates pharmaceutical supply, drug usage and compatibility, and drug-control regulations to medical and nursing staff and the hospital community.
  • The pharmacist provides sick-call coverage, covers additional shifts, and supports pharmacy areas requiring additional assistance.
  • The pharmacist works with minimal supervision, independently solves problems, and provides solutions for drug shortages, sick-call coverage, narcotic discrepancies, and computer downtime.
  • The pharmacist assists with reviewing and revising policies, procedures, and medication order sets for the Cerner system.
  • The pharmacist completes patient-monitoring profiles.
  • The pharmacist makes therapeutic recommendations according to nationally published guidelines and the hospital formulary.
  • The pharmacist participates in continuous-quality-improvement activities and pharmacy clinical programs.
  • The pharmacist adheres to therapeutic guidelines and recommends optimization of patient therapy.
  • The pharmacist participates in hospital programs for adverse drug reaction and medication-error reporting.
  • The pharmacist enforces adherence to the hospital formulary and applies therapeutic-interchange policies.
  • The pharmacist documents clinical interventions in the department-approved clinical documentation system.
  • The pharmacist trains pharmacists-in-training, newly hired pharmacists, and residents.
  • The pharmacist precepts and mentors pharmacy students and interns.
  • The pharmacist provides formal and informal drug education to pharmacy, medical staff, nursing, and ancillary services.
  • The pharmacist performs other related duties incidental to the work described.
Desired Qualifications
  • Three years of oncology experience or completion of an oncology residency is preferred.
