Requirements
  • The nurse practitioner must graduate from an accredited School of Nursing program, hold registered nurse licensure, complete an accredited Nurse Practitioner program with a Master’s Degree in Nursing, and have at least two years of advanced practice provider experience in Cardiology.
  • The physician assistant must graduate from a program accredited by the Accreditation Review Commission on Education for the Physician Assistant, pass the Physician Assistant National Certifying Exam, hold PA state licensure, maintain National Commission on Certification of Physician Assistants certification, and have at least two years of experience working in Cardiology as a physician assistant.
  • An unrestricted professional license and Drug Enforcement Administration registration are required for each state in which the practitioner works, where applicable.
  • The role requires Cardiology experience.
Responsibilities
  • Provide services to nursing center residents in collaboration with the AlignMed Partners physician.
  • Provide cardiovascular-care consultation and education to center nursing staff.
  • Diagnose patients, prescribe treatment, and provide follow-up during wellness and illness.
  • Initiate referrals and implement total care plans involving other health disciplines.
  • Coordinate community care and oversee patient case management.
  • Provide patient and family teaching and education.
  • Provide cardiac care consultation to medical teams, patients, families, nursing, social work, and rehabilitation therapy staff.
  • Serve as a role model to facility nursing, therapy, and medical practitioners regarding cardiovascular-disorder care.
  • Diagnose and treat cardiovascular disease, prevent complications, and coordinate cardiovascular care at the center and during transitions of care.
  • Manage continuity of care from hospitalization through discharge and home, using clinical guidelines, scope of practice, and physician direction.
  • Collaborate with the Director of Nursing on center-specific goals and priorities and with the Medical Director or primary care advanced practice providers on patient-specific care.
Desired Qualifications
  • At least two years of experience working with adult or geriatric populations is preferred.
