Requirements
- A Doctor of Pharmacy degree or a Bachelor's degree in Pharmacy with equivalent experience.
- One to two years of hospital experience as a pharmacist.
- A State of Wisconsin license as a pharmacist.
- Basic Life Support certification within three months of hire.
- Advanced Cardiac Life Support certification within six months of hire.
Responsibilities
- Provide pharmaceutical services to SPH inpatients and outpatients and to satellite clinics.
- Enter medication orders into or perform final quality control checks in the Meditech computer system, and initial order sheets to indicate pharmacist review.
- Review new orders for appropriateness, duplication of therapy, drug-drug interactions, and drug allergies.
- Resolve drug therapy problems by intervening with the appropriate physician or health-team member and change orders when appropriate.
- Clarify incomplete, inaccurate, or unclear orders with the appropriate physician or health-team member.
- Perform dosage calculations and calculate and pursue dosage adjustments for patients with impaired renal or hepatic function.
- Perform and document therapeutic interchanges or formulary substitutions on appropriate agents.
- Document clinical interventions in the computer system.
- Review patients' computerized medication profiles and intervene when drug therapy problems are identified.
- Provide drug information for requests immediately affecting patient care.
- Perform drug monitoring consultations, including pharmacokinetic consultations.
- Manage total parenteral nutrition orders by reviewing and documenting laboratory results and performing necessary interventions.
- Perform therapeutic drug monitoring by reviewing and assessing appropriate laboratory results and drug levels, including for warfarin, anticonvulsants, and digoxin.
- Review patients' laboratory results and microbiology cultures and intervene with the appropriate physician when necessary.
- Perform and document first-dose teaching, diabetes teaching, warfarin teaching, and discharge counseling.
- Guide and perform final quality control checks of technical duties, including drug selection, preparation, packaging, labeling, and unit-dose cart checks.
- Prepare or supervise and check the preparation of parenteral sterile products, including injections, intravenous admixtures, and total parenteral nutrition.
- Instruct patients and healthcare professionals on proper drug administration methods and precautions or side effects.
- Ensure timely delivery of medications to all patient populations served.
- Assist with training new department employees.
- Provide drug information and pharmacotherapy recommendations to other healthcare professionals.
- Maintain proper drug storage, record keeping, and utilization data as required by state and federal regulations and good practice standards.
- Maintain current working knowledge of pertinent state and federal laws and regulations and hospital and pharmacy department policies and procedures governing hospital pharmacy practice.
- Inspect drug stocks at designated storage areas to verify storage conditions, expiration dates, quantities, and security.
- Read professional literature, local policies and procedures, and professional practice standards to stay current on new drug entities, drug precautions, drug shortages, recalls, and legal requirements of pharmacy practice.
- Perform as a Department of Pharmacy member of the hospital Code Blue cardiopulmonary resuscitation team.
- Document and report adverse drug reactions.
- Participate in designated hospital committees.
- Provide direct supervision of one to three pharmacy technicians and one intern or student at any time.
- Provide community service presentations, drug information, and prevention programs to consumer and patient groups.
