Requirements
  • A master's degree from an accredited school of social work and two years of professional social work practice are required, or an equivalent combination of education and experience.
  • State licensure as defined by the State Board for Social Work, State Education Department, Division of Professional Licensing Services is required upon hire.
  • Licensed Master Social Worker licensure is required upon hire.
Responsibilities
  • Coordinate continuity of care and discharge planning for the Pediatric Inpatient Psychiatry unit.
  • Provide general oversight and supervise social work staff and the delivery of services on Pediatric Inpatient Psychiatry.
  • Provide consultation support on other pediatric disposition cases in the system as appropriate and in conjunction with the Social Work Manager for Inpatient Psychiatry.
  • Develop and maintain administrative liaisons with community agencies and referral sources to facilitate continuity of care and timely discharge of patients from the inpatient service.
  • Provide social work services to pediatric patients and their families while boarding for admission on Pediatric Inpatient Psychiatry.
  • Participate as a member of the interdisciplinary treatment team in developing and implementing patients' treatment and discharge plans.
  • Provide psychosocial and family assessments and coordinate the discharge planning process.
  • Provide individual, family, or group treatment that contributes to implementing comprehensive discharge plans.
  • Provide social work treatment in response to patient and family psychosocial problems that may impede care or discharge.
  • Develop interagency relationships and mobilize community resources on behalf of patients and their families.
  • Supervise the delivery of social work services on the Pediatric Inpatient Psychiatry Unit and implement policies and procedures regarding patient care, discharge planning, and professional practice in collaboration with the Social Work Manager, Social Work Division, Department, and Hospital.
  • Participate in recruitment, training, and evaluation of inpatient social work staff in collaboration with appropriate interdisciplinary members of the program.
  • Conduct weekly meetings of inpatient social work staff, including informal or formal individual supervision or supervision within treatment team settings.
  • Collaborate regularly with the Pediatric Inpatient Leadership Team to identify and resolve problems within the delivery of care system, with special emphasis on discharge planning.
  • For CPEP pediatric boarders awaiting inpatient placement, identify and coordinate a safe, timely, and comprehensive discharge plan with the treatment team and patients' families, including safety planning and continuity of care.
  • Identify and coordinate with outpatient providers and agencies to facilitate continuity of care and discharge planning.
  • Provide education to patients and families regarding internal and community resources and referrals, and advocate for and support patients and families interacting with complex social service agencies.
  • Document interventions in compliance with Division, Department, and Hospital policy.
  • Develop administrative liaisons with community agencies, including OMH, CPS, SPOA, and OPWDD, and referral resources to facilitate patient movement from inpatient services to home or community settings.
  • Participate in regularly scheduled meetings with community agencies for community planning and support.
  • Attend Social Work Division and Department staff meetings as required.
  • Participate in Social Work Division activities, including timely completion of time reporting and other Hospital or Division forms, mandatory in-service training programs, and UHS reviews.
  • Treat patients, families, trainees, and coworkers sensitively with appropriate attention to cultural differences and participate in available cultural competence events and training appropriate to job duties.
  • Provide social work coverage and backup as needed during absences of Pediatric Inpatient Psychiatry social work staff.
  • Perform other duties as requested and assigned.
