Requirements
  • An active Registered Nurse license in good standing is required.
  • At least 2 years of clinical experience is required.
  • Strong communication, organizational, and critical thinking skills are required.
  • The ability to work independently and as part of a multidisciplinary team is required.
  • Current cardiopulmonary resuscitation certification for Health Care Providers is required.
Responsibilities
  • Conduct comprehensive patient assessments, including physical, emotional, and psychosocial evaluations.
  • Support symptom management, medication reconciliation, and patient education.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.
  • Facilitate advance care planning discussions and goals-of-care conversations as appropriate.
  • Coordinate outpatient services, including scheduling, prior authorizations, and follow-up care.
  • Communicate test results and care updates to patients and families with empathy and clarity.
  • Provide triage support for urgent medical issues during clinic hours and on a rotating basis.
  • Maintain accurate documentation in the electronic health record.
  • Assist the inpatient service with triage and phone coverage as needed.
  • Build trusting relationships with patients and families by offering emotional support and guidance.
Desired Qualifications
  • Clinical experience in oncology, hospice, or palliative care is preferred.
  • Familiarity with electronic health record systems and outpatient workflows is preferred.
