- Receive and respond to referrals from hospitals, home health agencies, physicians, case managers, skilled nursing facilities, and other healthcare professionals.
- Assess patient eligibility for hospice services by reviewing medical records, conducting clinical evaluations, and communicating with physicians and families.
- Educate patients and families about what hospice care includes, how it works, and why it matters.
- Complete necessary documentation and coordinate the intake and admission process, ensuring that legal, clinical, and payer requirements are met.
- Serve as a liaison between referral sources and the hospice care team to ensure seamless handoffs, clear communication, and rapid responses to needs.
- Make daily visits to hospitals and facilities while maintaining visibility and accessibility with discharge planners, case managers, and physicians.
- Build and maintain relationships with referral partners and identify new outreach opportunities to increase awareness of hospice services.
- Provide coverage for the Dickson area.
- A Bachelor of Science in Nursing with five or more years of hospice experience is preferred.
- Prior experience as a discharge planner, case manager, or educator is preferred.
