Requirements
  • Bachelor’s Degree in relevant field.
  • Registered Nurse Licensing (RN).
  • Minimum 2 years of relevant experience; 1 year of case management experience is preferred or 2 years of hospital nursing experience.

Responsibilities
  • Coordinate the overall interdisciplinary plan of care for patients from admission to discharge.
  • Monitor care and act as a liaison between patients and families, healthcare personnel, and insurers.
  • Evaluate patient needs and available resources, and recommend and facilitate the plan for the best outcome.
  • Develop discharge plans using available resources to meet ongoing patient needs and encourage compliance with medical advice.
  • Identify patient care issues and suggest revisions to or new clinical pathways to improve quality of care.
  • Facilitate care for assigned extended-recovery, outpatient-observation, and inpatient admissions, including care progression, consultations, testing, social-service intervention, and individual discharge planning.
  • Monitor resource consumption and capture avoidable days.
  • Enter ancillary notes using care-facilitation templates.
  • Obtain needed orders and facilitate delivery of clinical and community services to patients and families through effective utilization of available resources.
  • Attend and facilitate daily multidisciplinary huddles and meetings addressing progression of care.
  • Ensure the appropriateness and cost-effectiveness of patient care plans based on diagnosis-related groups.
  • Collaborate with physicians to develop patient care plans and review medical needs for continued hospital services and resource consumption.
  • Use case-manager nurse-driven protocols and request physician orders for items outside those protocols.
  • Provide required Medicare documents to patients or proxies, including discharge Important Message from Medicare and Code 44 notification documents.
  • Process Quality Improvement Organization Medicare appeals.
  • Facilitate acute-care transfers, including psychiatric transfers.
  • Attend and report on assigned length-of-stay 10-day outliers during complex case review.
  • Communicate daily with management about observation outliers, care-transition issues, and discharge barriers.
  • Identify patient risk factors, obstacles to care and discharge, and readmission risk.
  • Evaluate plans of care through chart review and patient interviews, and collaborate with the medical team to facilitate patient movement through the system.
  • Educate patients and families about progression of care.
  • Serve as a liaison among patients, families, and healthcare personnel to ensure necessary care is provided promptly, effectively, and responsibly.
  • Promote quality care by ensuring patients receive medically appropriate services in the appropriate status and stay standards.
  • Facilitate regulatory notifications and obtain patient signatures according to policy.
  • Maintain knowledge of insurance reimbursement policies.
  • Plan and facilitate discharge and transition plans that meet patients’ physical, social, and emotional needs.
  • Adhere to University and unit-level policies and procedures and safeguard University assets.
Desired Qualifications
  • 1 year of case management experience is preferred.
