Requirements
  • Bachelor’s degree in Human Services, Social Work, Education or a related Social Science field
  • 2 years of experience in case management, job development or crises intervention with youth
  • Maintain certificates required for the position including but not limited to First Aid and CPR
  • Valid Texas Driver’s license with a clear driving record and reliable transportation
  • Pass a pre-employment drug screen and random drug screens
  • Provide proof of work eligibility status upon request
  • Pass a pre-employment and biennial criminal background and motor vehicle check (fingerprints needed if applicant has lived out of state within the past 5 years)
  • Bilingual. Spanish required.
Responsibilities
  • Assess the personal needs of PAL clients and help to obtain resources to meet those needs by providing case management services
  • Advocate with human services, health, and court and criminal justice systems to resolve issues facing clients
  • Assists youth in crisis by providing de-escalation for situation and developing plan with youth to move out of state crises
  • Develop detailed Individual Service Plans for each client and meet with them regularly to assess progress and update the plans
  • Maintain timely and accurate file documentation and data entry that meet the requirements of the PAL program
  • Follow all HHS guidelines and policies concerning PAL TFSS services
  • Demonstrate a working knowledge of all contracts and agency policies and procedures
  • Meet all federal and state regulatory guidelines and standards that are applicable to this position
  • Participate/lead PAL program weekly, monthly events and activities
  • Work evenings or weekends as needed or requested by Program Director
  • Implement BCFS HHS safety protocols including evacuating with children and other staff in case of an emergency
  • Get approval for services through authorization form (2054) from HHS PAL staff
  • Show knowledge of community agencies and resources
  • Maintain confidentiality in all areas of the service population and program operations
  • Maintain BCFS HHS professional and ethical standards of conduct outlined in the employee handbook including demonstrating respect for agency staff, the service population, and community members and complying with the required dress code, at all times
  • Attend all required program trainings as applicable
  • Record all services and activities in a complete, concise, and timely manner utilizing program forms and Salesforce database
  • Report on any suspected child abuse and neglect to the program director and law enforcement agency
  • Ability to maintain confidentiality with clients and staff
  • Demonstrate the ability to: Respond sensitively and competently to the service population’s cultural and socio-economic characteristics, Communicate effectively in writing and verbally in English, Work in a fast-paced environment, Maintain computer literacy required to meet the responsibilities of the position, Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team, Travel as needed
  • Other job duties as assigned per grant requirements.
