Requirements
  • A high school diploma with one year of experience in human services, such as social work, psychology, special education, or a related field, is required.
  • One year of administrative experience is required.
  • At least two years of experience working with individuals diagnosed with intellectual disability is required.
  • AMAP, SCIP-R, and First Aid/CPR certification is required.
  • A valid driver’s license is required, including the ability to drive a large vehicle such as a wheelchair-accessible vehicle or 15-seater van.
  • The role requires respect and consideration for residents being served.
  • The employee must be able to work Sunday through Thursday from 1:00 PM to 9:00 PM, subject to the Program Manager’s discretion.
Responsibilities
  • Coordinate and implement in-house programming, including assessments, goal planning, daily progress evaluations, monthly resident reviews, recreational-program monitoring, community-resource development, and maintenance of the van trip log.
  • Use independent judgment and discretion to ensure individuals receive a high standard of service and that their day-to-day needs are met.
  • Ensure services are delivered according to each individual’s plans.
  • Assess the holistic needs of individuals and families, including food, clothing, and shelter, and connect them with appropriate internal and external services.
  • Make referrals, follow up on referrals made on behalf of individuals and families, and respond to referrals made to the program.
  • Participate in cross-program service reviews supporting service-delivery planning, coordination, and closure.
  • Organize records and ensure that they remain up to date.
  • Participate in interdisciplinary team and staff meetings.
  • Create meeting agendas with the Program Manager, including committee reports, announcements, and staff concerns.
  • Collaborate with consultants, especially the psychologist, on programming.
  • Follow through on recommendations for the U.R. committee and attend U.R. meetings when delegated.
  • Complete and maintain investigation certification related to incidents within the Developmental Disabilities Services program.
  • Ensure that life-safety safeguards for individuals are implemented and followed.
  • Maintain and submit timely, accurate records and reports concerning individuals’ health and safety and program quality.
  • Submit, update, and close all program incidents.
  • Maintain compliance with the Office for People With Developmental Disabilities and the Justice Center regarding incidents.
  • Make records available for IPR and UR review.
  • Prepare Program Specialist staff schedules, including vacation, personal, and sick leave, and maintain coverage during emergencies and staff shortages.
  • Ensure staffing patterns comply with facility policy.
  • Conduct scheduled supervisor meetings regarding programming and staffing issues.
  • Evaluate staff work performance through observations and performance counseling in conjunction with the Program Manager.
  • Ensure individuals receive or are assisted with skill development in activities of daily living, domestic living, recreation, and socialization.
  • Assist the Program Manager in developing and implementing agency and program policy in accordance with Office for People With Developmental Disabilities and other funding-agency guidelines.
  • Conduct frequent rounds on all shifts and report to the Program Manager on safety, health requirements, the physical environment, individual care, and programming quality.
  • Track medical appointments and communicate with the central medical services team, nurse, doctors, and managers.
  • Ensure the accuracy of medication inventory, maintain medication records, and order refills when necessary.
  • Track and follow up on medical appointments, complete medical referral forms, and complete monthly medical-book reviews.
  • Coordinate supervisors’ transportation activities for day programming, recreation services, medical appointments, and special requirements.
  • Ensure transportation of individuals to and from programs, community-inclusion activities, medical appointments, home visits, and other activities as needed.
  • Ensure completion of the Staff Information Entry Form when escorting individuals to medical appointments.
  • Ensure timely receipt and ongoing availability of individuals’ needs, including toiletry items, clothing, and personal effects.
  • Prepare day-program progress reports for individual treatment-team reviews as assigned.
  • Distribute petty cash funds to staff or collect petty-cash receipts and submit them to the Program Manager.
  • Conduct monthly fire-evacuation drills with staff on all shifts.
  • Participate in admission and discharge reviews and make recommendations to the Program Manager.
  • Develop a monthly recreation schedule in consultation with the Program Manager.
  • Implement and monitor in-house and community recreational activities, providing hands-on assistance as needed.
  • Direct care as necessary.
  • Perform other assigned duties.
