Care is delivered by clinicians and therapists in patients’ homes, including an Advanced Illness Management model for those not yet hospice-eligible. The company differentiates itself with an integrated at-home care platform and strategic acquisitions, such as ProMedica’s home health, palliative, and hospice operations.
