Requirements
  • An active, unrestricted LPN/LVN license in good standing is required.
  • At least 2 years of patient- or community-facing support experience is required, such as care coordination, case management, member advocacy, health coaching, benefits navigation, or community health work.
  • Working knowledge of the U.S. healthcare system and Medicare is required.
  • The ability to work independently, manage a caseload, and maintain strong documentation is required.
  • A reliable computer, internet connection, and quiet space for patient calls are required.
  • The role requires maintaining confidentiality and complying with HIPAA, privacy, and applicable platform standards.
Responsibilities
  • Serve as the primary point of contact for patients after their intake visit.
  • Help patients understand and act on their care plan by scheduling appointments, finding in-network providers, and helping them use their Medicare benefits.
  • Resolve navigation issues involving portals, referrals, appointment logistics, forms, and provider communications.
  • Identify practical and social needs, including food, housing, and transportation, and connect patients with available resources.
  • Coordinate with clinical team members involved in a patient's care while providing non-clinical support only.
  • Document every patient interaction and support activity in the platform accurately and on time.
Desired Qualifications
  • Experience supporting older adults or patients with complex or serious illness is preferred.
  • Familiarity with Medicaid and other public programs is preferred.
  • Bilingual ability in Spanish or another language common among Medicare patients is preferred.
